The use of lymphaticovenous anastomosis in the surgical treatment of postmastectomy lymphedema

Abstract

Background. Postmastectomy lymphedema is a debilitating and progressive complication, with few treatment options, which considerably lowers breast cancer survivors’ quality of life. Its incidence varies from 0-3% after lumpectomy to 65-70% after radical mastectomy and radiotherapy. Objective of the study. The purpose of this study is to determine the effectiveness of lymphaticovenous anastomosis in the prevention and treatment of postmastectomy lymphedema in various stages of the disease. Material and Methods. For this literature review more than 50 scientific articles have been studied in great detail. Results. Lymphaticovenous anastomosis has proven to be efficient in the surgical management of postmastectomy lymphedema in the first and second stage together with conservative treatment or liposuction and in the third and four stage in conjunction with lymph node transfer or Charles procedure. The supermicrosurgical technique is more efficient in the incipient or moderate stages of the disease than in severe lymphedema. Conclusion. Lymphaticovenous anastomosis is a supermicrosurgical technique useful in both the prevention of postmastectomy lymphedema development and the prevention of lymphedema progression as well as its management in all stages of the disease.

Introducere. Limfedemul postmastectomie este o complicație debilitantă și progresivă, cu puține opțiuni de tratament, ce scade considerabil calitatea vieții supraviețuitorilor de cancer mamar. Incidența variază de la 0-3% după lumpectomie, la 65-70% după mastectomie radicală și radioterapie. Scopul lucrării. Scopul acestei lucrări este de a determina eficacitatea anastomozei limfatico-venoase în prevenirea și tratamentul limfedemului postmastectomie în diferite stadii de avansare ale bolii. Material și Metode. Pentru această sinteză a literaturii de specialitate au fost studiate minuțios peste 50 de articole științifice. Rezultate. Anastomoza limfatico-venoasă s-a dovedit a fi eficace în managementul chirurgical al limfedemului postmastectomie în stadiul 1-2 împreună cu un tratament conservator sau liposucție și în stadiul 3-4 în conjuncție cu transferul de nodul limfatic sau procedura Charles. Tehnica supermicrochirurgicală este mai eficientă în stadiile incipiente sau moderată ale bolii decât în cea severă. Concluzii. Anastomoza limfatico-venoasă este o tehnică supermicrochirurgicală utilă atât în prevenirea dezvoltării limfedemului postmastectomie, cât și în prevenirea agravării bolii și managementul acesteia în toate stadiile ei.
