Impactul asupra sănătății și cel socioeconomic al morbidității prin varicelă în Republica Moldova

Abstract

A fost studiată și analizată epidemiologic morbiditatea prin varicelă în Republica Moldova în perioada 1985-2017. Studiul dat este unul longitudinal descriptiv retrospectiv. În Republica Moldova, morbiditatea multianuală prin varicelă din 2003 până în 2016 a avut o tendință de creștere. 60,19% din respondenți au suportat varicela în forma ușoară, iar 39,18% – în forma medie. Din numărul total de pacienți internați în spital, 44,17% au suportat varicelă de gravitate medie, cei cu varicelă de gravitate medie și alte boli asociate au constituit 25,77%, gravitate severă cu complicații – 9,20%, gravitate severă asociată cu alte boli – 20,86%. Suma medie a cheltuielilor pacienților pentru tratamentul ambulatoriu al varicelei constituie 858,53 lei, iar suma medie a cheltuielilor pentru tratamentul celor internați în spital – 2522,53 lei. Media cheltuielilor totale constituie 1690,53 lei. Cheltuielile legate de cazul de deces alcătuiesc 2.812.449,97 lei. Cheltuielile pentru procurarea monovaccinului Varivax de 12.499.157,84 lei sunt de 1,64 ori mai mici decât cele anuale legate de morbiditatea prin varicelă în Moldova – 20.536.440,05 lei. Iar cheltuielile pentru procurarea tetravaccinului ProQuad, care constituie 7.971.208,25 lei, sunt de 2,58 ori mai mici decât cele anuale legate de morbiditatea prin varicelă. Cele mai mari cheltuieli per capita în urma morbidității prin această maladie se înregistrează în mun. Bălți – 41,9 lei, iar cheltuielile per capita pentru vaccinarea copiilor de 1 an în mun. Bălți sunt de 13,43 ori mai mici și constituie 3,12 lei.

The impact on the health and socio-economic impact of the morbidity of varicella in the Republic of Moldova There was studied and carried out the epidemiological analysis of chickenpox morbidity in the Republic of Moldova during 1985-2017 years. Results of the study demonstrated that chickenpox annual morbidity in Republic of Moldova from 2003 year until 2016 had an increasing tendency. 60,19% of the respondents had chickenpox in the mild form, and 39,18% of patients had chickenpox in the medium form. Out of the total of hospitalized patients 44.17% had chickenpox of moderate severity, patients with chickenpox of moderate severity in association with other diseases – 25,77%, patients who had chickenpox in severe form with complications – 9,20%, and 20,86% of patients with chickenpox in severe form in association with other diseases. The average expenditure per patient with chickenpox is $100,63. The costs associated with the fatal case are – 167.407,74$. The costs associated with the acquisition of monovaccineVarivax– 743.997,49$ are 1,64 times lower than the annual costs associated with the incidence of chickenpox in the Republic of Moldova, which is 1.222.407,15$. And the costs associated with the acquisition of the combined vaccine ProQuad, which is 474.476.68$ 2,58 times less than the annual losses associated with the incidence of chickenpox. The highest costs/losses per capita due to the incidence of chickenpox are recorded in the city of Balti $2,5, and the costs/expenses per capita for vaccination of children from 1 year in the city of Balti are 13,43 times less and amount to$0,19.

Влияние на состояние здоровья и социальноэкономические последствия заболеваемости ветряной оспой в Республике Молдова Изучен и проведён эпидемиологический анализ заболеваемости ветряной оспой в Республике Молдова в период 1985-2017 г. Данное исследование является лонгитудинальным дескриптивным ретроспективным. Результаты исследования показали, что в Республике Молдова многолетняя заболеваемость ветряной оспой с 2003 до 2016 года имела выраженную тенденцию к росту. 60,19% из респондентов переболели ветряной оспой в легкой форме, а 39,18% пациентов – в средней форме. Из общего числа пациентов, госпитализированных в больницу, 44,17% переболели ветряной оспой средней тяжести, пациенты, переболевшие ветряной оспой средней тяжести в ассоциации с другими заболеваниями – 25,77%, те, которые переболели ветряной оспой в тяжелой форме с осложнениями – 9,20%, и 20,86% это пациенты с ветряной оспой в тяжелой форме в ассоциации с другими заболеваниями. Средняя сумма расходов на одного пациента для амбулаторного лечения ветряной оспы составляет 51,1$. Средняя сумма расходов на лечение одного пациента, госпитализированного в больницу с ветряной оспой составляет 150,15$. Средняя сумма расходов на лечение одного пациента с ветряной оспой – 100,63$. Расходы, связанные с летальным случаем, составляют 167.407,74$. Расходы для приобретения моновакцины Варивакс– 743.997,49$, в 1,64 раза ниже, чем ежегодные расходы, связанные с заболеваемостью ветряной оспы в Республике Молдова, которые составляют 1.222.407,15$. А расходы на приобретение комбинированной вакцины ProQuad, которые составляют 474476,68$, в 2,58 раза меньше, чем ежегодные потери, связанные с заболеваемостью ветряной оспой. Самые большие затраты/потери на душу населения в результате заболеваемости ветряной оспой регистрируются в Бельцах – 2,5$, а расходы/затраты на душу населения для вакцинации детей от 1 года в Бельцах являются в 13,43 раза меньше и составляют 0,19$.
