dc.description.abstractScopul lucrării este elucidarea cauzelor şi frecvenţei complicaţiilor specifice postoperatorii la bolnavii maturi cu malformaţii cardiace congenitale (MCC) şi şuntare intracardiacă, şi aprecierea stării funcţionale a pacienţilor operaţi. Material şi metode: În lucrare sunt prezentate rezultatele studiului unui lot de 348 pacienţi cu vârste cuprinse între 18 şi 66 ani, dintre care 234 (67,2%) pacienţi au fost femei şi 114 (32,8%) – barbaţi, cu MCC cu şuntare intracardiacă. Rezultate: Complicaţiile pulmonare au alcătuit 61% de cazuri. Postoperator clasa funcţională III NYHA s-a înregistrat la 29 (8,4%) pacienţi şi clasa funcţională IV NYHA – la 25 (7,4%) pacienţi, restul fiind în clasa funcţională I şi II NYHA. Mortalitatea a alcătuit 1,15% (4 pacienţi). Concluzii: Tratamentul MCC este exclusiv chirurgical, intervenţia practicându-se de preferinţă până la apariţia hipertensiunii pulmonare. Indicaţiile la operaţii nu depind de vârsta bolnavului ci de dereglările hemodinamice şi gradul obstrucţiei pulmonare. Modernizarea tehnicilor chirurgicale şi a tehnicii de protecţie miocardică intraoperatorie a dus la scăderea complicaţiilor postoperatorii şi a mortalităţii la 1,15%.en_US
dc.description.abstractThe aim of study was elucidation of causes and frequency of specific postoperative complications in adult patients with congenital heart defects (CHD) and assessment of functional status of operated patients with intracardiac shunting. Material and methods: This work presents the results of a group of 348 patients aged between 18 and 66 years, of which 234 (67.2%) patients were women and 114 (32.8%), were men with CHD and with intracardiac shunting. Results: Postoperative pulmonary complications were registered in 61% of cases. Functional class III NYHA was diagnosed postoperatively in 29 (8.4%) patients and NYHA functional class IV in 25 (7.4%) patients, the rest were in NYHA functional class I and II. Mortality was 1.15% (4 patients). Conclusions: Curative treatment of CHD is exclusively surgical, intervention being preferable performed before pulmonary hypertension development. Indications for operations do not depend on the patient's age and hemodynamic disturbances but it depends on degree of pulmonary obstruction. Upgrading surgical techniques and intraoperative myocardial protection technique have reduced postoperative complications and mortality to 1.15%.
