dc.description.abstractBackground. Liver transplantation is currently the first priority treatment for patients with advanced chronic liver failure. Early complications are those within the first 6 months after surgery Objective of the study. To evaluate early technical complications (arterial, biliary), medical (cardiovascular, respiratory, neurological, renal and infections) and liver graft complications in transplant patients. Material and Methods. A descriptive, retrospective cohort clinical study was performed in the period 2013-2020 within the transplant program in the Republic of Moldova, 57 liver transplant interventions were performed. The study selected 53 transplant patients, in whom 55 liver transplants were performed (2 retransplanted patients), 16 - from a living donor, 39 - from a brain-dead donor, aged> 18 years. Results. Early technical complications were determined in 19% (10/53) patients, hepatic artery thrombosis, in 6/53 patients (11%). Early medical complications were detected in 43 patients (81%), neurological complications 33/53 (62%) and respiratory complications 31/53 (58.5%), caused by immunosuppressive treatment. After liver transplantation, liver transplantation was impaired in 2/53 patients (4%), both of whom died. Conclusion. The risk of developing early post-surgical complications is associated with the patient's perioperative conditions, the quality of the donated liver, the quality of the surgical procedure performed on both the donor and the recipient. Introducere. Transplantul de ficat este în prezent tratamentul de elecție pentru pacienții cu insuficiență hepatică cronică avansată. Complicaţiile precoce reprezintă complicaţiile intervenite la pacienţii transplantaţi în decursul primelor 6 luni în urma intervenţiei. Scopul lucrării. Evaluarea complicaţiilor precoce tehnice (arteriale, biliare), medicale (cardiovasculare, respiratorii, neurologice, renale şi infecţiile) şi a complicaţiilor grefei hepatice la pacienţi transplantaţi. Material și Metode. A fost realizat un studiu clinic de cohortă, descriptiv, retrospectiv în perioada 2013-2020, în cadrul programului de transplant din Republica Moldova, efectuându-se 57 de intervenţii de transplant hepatic. În studiu, au fost incluși 53 de pacienți transplantaţi, la care s-au efectuat 55 de transplanturi hepatice (2 pacienţi retransplantaţi), 16 – de la donator viu, 39 – de la donator în moarte cerebrală. Rezultate. Complicaţiile precoce tehnice s-au determinat la 19% (10/53) pacienţi, tromboza arterei hepatice, la 6/53 pacienţi (11%). Complicațiile precoce medicale s-au depistat la 43 de pacienţi (81%), complicaţiile neurologice 33/53 (62%) şi respiratorii 31/53 (58,5%), cauzate de tratamentul imunosupresiv. După transplant hepatic afectarea grefei hepatice la 2/53 pacienţi (4%), ambele cazuri soldate cu deces. Concluzii. Riscul dezvoltării complicaţiilor postoperatorii precoce este asociat cu condiţiile perioperatorii ale pacientului, calitatea ficatului donat, calitatea procedurii chirurgicale efectuată atât la donator, cât şi recipient.en_US
