Abstract: Rezumat. Introducere. Boala parodontală continuă să reprezinte o problemă majoră de sănătate. Cercetările recente au scos în evidență și rolul factorilor de risc, cum ar fi: formarea radicalilor liberi (RL), care provin din mediul interior și din mediul exterior. Superoxid dismutaza (SOD) este o metaloenzimă antioxidantă, implicată în sistemul de apărare împotriva ROS. Prezentă atât în interiorul cât și în afara membranelor celulare, SOD, împreună cu glutation peroxidaza şi catalaza, fac parte din sistemul primar intern de apărare anti-oxidantă al organismului și joacă un rol critic în reducerea stresului oxidativ, implicat în dezvoltarea unei game largi de afecțiuni degenerative, ce pun viata în pericol. În acest context, acțiunea antioxidantă a ozonului medical merită luată în consideraţie. Actualitatea aplicării ozonului medical în cazul proceselor degenerative în regiunea oro-maxilo-facială este determinată de dereglarea statusului antioxidant, implicat în dezvoltarea PMC. Astfel, aspectele utilizării ozonului medical la pacienții cu PMC necesită o studiere mai aprofundată. Material și metode. Pentru acest studiu, au fost selectați 96 de pacienți, femei și bărbați, cu vârsta cuprinsă între 41 și 73 de ani, cu diagnosticul de PMC forma gravă. Pacienții au fost divizați în două loturi – lotul de control (52 de pacienți), căruia i-a fost administrat un tratament parodontal clasic, nechirurgical (scalarea și planarea radiculară, SRP) și lotul de studiu (44 de pacienți), căruia i-a fost administrat un tratament parodontal clasic, nechirurgical, complementat cu ozon medical, aplicat sistemic în forma de autohemoterapie majoră (AHTM). Evaluarea nivelului seric al markerului stresului oxidativ SOD, s-a realizat pe baza analizei sangvine, prin metoda fotometrică (enzimatică). Examinarea pacienților s-a efectuat în dinamică: până la tratament, după tratament și la intervale de 1, 3 și 6 luni de la tratament. Rezultate. Numărul pacienților cu nivelul seric al markerului SOD în normă, a crescut de la 11% la 84% (p<0,001) imediat după tratament și la 95 % (p<0,001) – la 1 lună după tratament, ajungând la 100% (p<0,001) la 3 luni după tratament și menținându-se în normă la 6 luni după tratament. Concluzii. Modularea nivelului seric al superoxid dismutazei a redus stresul oxidativ, implicat în patogeneza parodontitei marginale cronice, forma gravă, și a condus la echilibrarea homeostaziei redox.
Abstract. Introduction. Periodontal disease continues to be a major health problem. Recent research has also shown the important role of risk factors, such as the formation of free radicals (FR), which can be endogenous and exogenous. The superoxide dismutase (SOD) is an antioxidant metalloenzyme involved în the defense system against reactive oxygen spices (ROS). Together with glutathione peroxidase, ROS is part of the primary antioxidant system of defense of the body, which can be found inside and outside the cells’ membranes and has a critical role în the reduction of oxidative stress that causes the development of a wide range of degenerative diseases, which endanger life. în this context, it is important to consider the antioxidative action of medical ozone. The relevance of using medical ozone în degenerative processes of the oral maxillofacial region is determined by the disruption of the anti-oxidative status, which is involved în the development of CMP. Thus, the use of medical ozone în patients with CMP requires further and thorough research. Material and methods. For this study, 96 patients, men and women, aged 41-73, diagnosed with severe CMP were selected. The patients were divided into two lots – the control lot of 52 patients, which received a classical non-surgical periodontal treatment (scaling and root planing treatment (SRP); and the research lot of 44 patients, which received the same treatment complimented with medical ozone în the form of major autohaemotherapy (MAHT). Based on blood analysis, the photometric method was used, în order to evaluate serum level of the oxidative stress marker SOD. The examination of patients was carried out by observing the dynamics of the results before the treatment, after the treatment and at 1, 3, and 6 months after the treatment. Results. The number of patients having a normal serum level of the SOD marker, increased from 11% to 84% (p<0.001) immediately after the treatment, and up to 95% (p<0.001) în 1 month after the treatment, attaining the maximum of 100% (p<0.001) în 3 months, and still keeping în the same condition în 6 months after the treatment. Conclusions. Modulation of the serum level of the superoxide dismutase has reduced oxidative stress, involved în the pathogenesis of the severe chronic marginal periodontitis and led to the balance of the redox homeostasis.
