dc.description.abstractThe clinic case is represented by a polytrauma patient after a car crash with a liver injury of the IV degree. After the surgery intervention on liver (hepatorraphy and mesh packs) the patient developed a giant subdiafragmal biloma confirmed by USG and CT. The biloma was ECO-guided punctured, then drained in the VII intercostal space on the right side with a bilumen drainage. During the procedure neither complications were seen. The level of the bile was 200- 250 ml/24 hours. Fistulography made in three days after the drainage showed a considerable decrease of the subdiafragmal leak. The external bile fistula closed at the 14th day. The patient was hospitalized for 58 days in good physical condition. Cazul clinic este reprezentat printr-un pacient politraumatizat după un accident rutier cu o leziune de ficat de gradul IV. După intervenţia chirurgicală pe ficat (hepatorafie + meşiere perihepatică) pacientul dezvoltă un bilom gigant subdiafragmal confirmat prin USG şi la CT. Bilomul a fost punctat ECOghidat, apoi drenat în spaţiul VII i/costal pe dreapta cu dren bilumen. Complicaţii în timpul procedurii n-am avut. Debitul bilei a fost de 200-250 ml/24 ore. Fistulografia efectuată la 3 zi după drenare ne demonstreză micşorarea considerabilă a colecţiei subdiafragmale. Fistula biliară externă s-a închis la 14 zi. Pacientul s-a aflat în staţionar 58 zile şi s-a externat în stare satisfăcătoare.en_US
