Abstract: În lucrarea prezentată se analizează rezultatul tratamentului endovasclar a unui pacient cu tromboză venoasă profundă a membrelor inferiore embologenă. Pacientului i-a fost implantat cava-filtru şi i-a fost efectuată tromboliza ghidată pe cateter. Decizia de a efectua sau nu tromboliza ghidată pe cateter la pacienţii cu tromboză venoasă profundă a membrelor inferioare este bazată pe tipul şi forma trombozei. În primul rând, intervenţiile chirurgicale sunt indicate pacienţilor cu trombi fl otaţi şi cu risc înalt embologen. Trombectomiile sunt indicate pacienţilor cu prognostic favorabil de evoluţie a bolii, dacă activarea procesului patologic este posibilă. Cava-filtrele sunt recomandate pacienţilor diagnosticaţi cu trombemolism pulmonar recurent, cu prognostic nefavorabil a bolii şi deasemenea celor cu patologii concomitente severe. Controlul ultrasonografic este obligator la toate etapele de management The present work deals with analysing the outcomes of endovascular management carried out in a patient diagnosed as having embologenic thrombosis of the lower-limb deep veins. The patient received a cava filter implanted and catheterdirected thrombolysis. The decision of whether or not to perform a catheter-directed thrombolysis in patient presenting with thrombosis of deep veins of the lower extremities was made based on the type and and pattern of the thrombus concerned. First of all, such surgery was indicated primarily for those diagnosed with a floating, embolus-hazardous thrombus. Thrombectomy was carried out in the patients with a favourable prognosis of the underlying disease, if activation of the pathological process was possible. Cava filters are recomended to the patients diagnosed as having a recurrent embolism, with an unfavourable prognosis of the course of the underlying disease, as well as to those presenting with a severe concomitant pathology. Ultrasonography-guided control is obligatory at all stages of management. В представленной работе анализируются результат эндоваскулярное лечение пациентa с эмбологенным тромбозом глубоких вен нижних конечностей. Пациенту установлен кавафильтр. При тромбозе глубоких вен нижних конечностей показания к эндоваскулярному лечение определялись в зависимости от типа и характера тромба. В первую очередь оно показано пациентам с флотирующим, эмболоопасным тромбом. Тромбэктомия выполнялась больным с благоприятным прогнозом течения основного заболевания, если возможна активизация. Кавафильтр устанавливался больным с рецидивирующей эмболией, при неблагоприятном прогнозе течения основного заболевания, а также при наличии тяжелой сопутствующей патологии. На всех этапах лечения обязателен ультразвуковой контроль.
