Abstract: The study group, under which this research has been carried out, is made up of 47 patients who had been diagnosed with cirrhosis of different etiologies and significant splenomegaly within the portal hypertension, complicated with severe hypersplenism. It was noted that before surgery, in all patients was present the severe hypersplenism syndrome, in 48.9 % of the patients was present the hepatocellular insufficiency syndrome, in 27.6% of the patients was present the cholestasis syndrome, in 57.9 % of the patients was present cytolytic syndrome. After surgery, there was an improvement of the blood count indices by increasing the number of red blood cells, platelets, leukocytes and hemoglobin in all patients, was also noticed an improvement of the liver synthetic function with a statistically significant increase of prothrombin and fi brinogen. We also observed clinical improvement and the decrease of the frequency of complications of liver cirrhosis. The surgical approach showed the benefits in the clinical and laboratory evolution of patients with cirrhosis of different etiology and splenomegaly secondary to portal hypertension complicated with severe hypersplenism.
Представлены результаты исследования 47 пациентов с диагнозом цирроз печени различной этиологии и значительной спленомегалией, в рамках портальной гипертензии, осложненной тяжелым гиперспленизмом. До операции, в 48,9% случаев отмечен синдром печёночной недостаточности, в 27,6% случаев присутствовал синдром холестаза и в 57,9% случаев – цитолитический синдром. После операции было установлено улучшение общего анализа крови, с повышением количества эритроцитов, тромбоцитов, лейкоцитов и гемоглобина у всех пациентов, а также замечено улучшение синтетической функции печени со статистически значимым увеличением уровня протромбина и фибриногена. Также было обнаружено улучшение клинических проявлений и уменьшение осложнений цирроза печени. Хирургический подход к пациентам с циррозом различной этиологии и спленомегалией в рамках портальной гипертензии, осложненной тяжелым гиперспленизмом, приносит пользу в эволюции клинико-параклинических проявлений.
