Abstract: In many malignant tumours the lymphatic system represents the main path for metastatic dissemination of the tumor cells in the lymph nodes. As is well known, the status of the lymph nodes is the prognostic factor in numerous human tumors and has great importance in establishing of the therapeutic strategy. Nevertheless, recent scientific data about lymphatic metastases, especially in the early stages of the tumoral proliferation, is insufficient. It is known that metastases of the tumour cells involve blood and lymphatic vascular systems. Tumoral cells influence these systems via secretion of growth factors necessary for formation of certain vascular networks which may facilitate the spread of metastasis. It is not yet clear, however, whether tumor cells invade the existent peripheral lymphatic vessels or invade the newly-created vessels which are formed during tumoral lymphangiogenesis. This is due to the later introduction of specific markers for lymphatic endothelium, insufficient knowledge of the factors which are implicated in the lymphangiogenesis, and the absence of experimental models which can permit the quantitative evaluation of metastases in the lymph nodes. Molecular control of the tumoral lymphangiogenesis and knowledge of the mechanism of lymphatic tumoral metastasis will offer new therapeutic tactics in treating human cancer. The aim of the study was the critical analysis of the literature regarding the structure, functions, and development of lymphatic vessels and their importance in tumoral pathology. Current understanding of the roles of lymphangiogenic factors, the mechanism of lymphatic metastases, and the inhibited therapeutic opportunity of lymphangiogenesis will also be presented.
Лимфатические сосуды представляют собой главный путь метастатической диссеминации опухолевых клеток в региональные лимфатические узлы. Состояние лимфатических узлов является хорошо известным фактором прогноза онкологических заболеваний и имеет решающее значение при выборе тактики лечения. Тем не менее в современной научной литературе недостаточно данных, проливающих свет на процесс лимфатического метастазирования, особенно на ранних стадиях его развития. Диссеминация опухолевых клеток через кровеносное и лимфатическое русла на сегодняшний день является общепризнанным фактом. Злокачественные клетки способны воздействовать на кровеносные и лимфатические сосуды с помощью выделяемых фактров роста, вызывая тем самым возникновение собственной сосудистой сети, призванной обеспечить рост и инвазию опухоли. До сих пор точно не установлено: проникают ли опухолевые клетки в существующие лимфатические сосуды или же в сосуды, возникшие в результате лимфангиогенеза. Такое состояние дел объясняется поздним появлением иммуногистохимических маркеров для эндотелиальных клеток лимфатических сосудов, недостаточным изучением факторов, вовлеченных в процесс лимфангиогенеза, а также отсутствием экспериментальной модели, позволяющей дать количественную оценку метастазов в лимфатических узлах. Понимание механизмов опухолевого лимфангиогенеза и лимфатического метазтазирования вплоть до молекулярного уровня позволило бы существенно продвинуться в лечении онкологических заболеваний. Автором статьи дан критический анализ научной литературы с точки зрения развития, структуры и функций лимфатических сосудов, а также их роли в онкологической патологии. Современные видения касательно роли факторов лимфангиогенеза, механизма лимфатического метастазирования и потенциальных терапевтических возможностей ингибирования лимфангиогенеза нуждаются в дальнейшем изучении.
