Abstract: Introducere: Sediul atipic, unde poate apărea ulcerul duodenal perforat şi astăzi continuă a fi o problemă actuală. Scopul: A studia incidenţa şi particularităţile de diagnostic şi tratament ale ulcerului duodenal postbulbar perforat. Material şi metode: Lotul de studiu cuprinde 24 bolnavi, spitalizaţi de urgenţă şi operaţi pentru ulcer postbulbar perforat în perioada aa. 2000-2019, selectaţi din totatul de 1094 pacienţi trataţi chirurgical pentru ulcer duodenal perforat. Raportul bărbaţi/femei a fost de 5/1 şi vîrsta cuprinsă între 41-60 ani. Intervenţiile chirurgicale efectuate la cei 24 pacienţi au fost: suturarea ulcerului- 18 cazuri; excizia ulcerului cronic cu piloroduodenoplastie şi vagotomie trunculară bilaterală- 6 cazuri. Rezultate: Incidenţa ulcerului postbulbar perforat a constituit 2,19% cazuri. În 18 cazuri simptomatologia clinică, radiologică şi endoscopică a fost caracteristică ulcerului duodenal perforat şi nu a impus dificultăţi de diagnostic. La 6 pacienţi semne clinice evidente lipseau, diagnosticul fiind stabilit doar laparoscopic, constatîndu-se o perforaţie acoperită. În marea majoritate a cazurilor- 17 (70,8%) leziunea ulceroasă a fost situată pe segmentul duodenal D1. Concluzii: Incidenţa ulcerului duodenal postbulbar perforat este în creştere, în studiul nostru cu o pondere de 2,19% şi necesită o vigilenţă sporită atît din punct de vedere diagnostic, cît şi intraoperatoriu.
Introduction: The atypical headquarters, where the perforated duodenal ulcer may appear continues to be a current problem. The aim: To study the incidence and particularities of the diagnosis and treatment of perforated postbulbar duodenal ulcer. Material and methods: The study group comprises 24 patients, hospitalized urgently and operated for perforated postbulbar ulcer during 2000-2019, selected from the total of 1094 surgically treated patients for perforated duodenal ulcer. The male / female ratio was 5/1 and the age was 41-60 years. Surgery in 24 patients was: ulcer suturing - 18 cases; excision of chronic ulcer with piloroduodenoplasty and bilateral truncular vagotomy - 6 cases. Results: The incidence of perforated postbulbar ulcer was 2.19%. In 18 cases, clinical, radiological and endoscopic symptomatology was characteristic of the perforated duodenal ulcer and did not pose diagnostic difficulties. In 6 patients, obvious clinical signs were missing, the diagnosis being determined only laparoscopically, with a perforation covered. In the vast majority of cases - 17 (70.8%) of the ulcerous lesion was located on the duodenal segment D1. Conclusions: The incidence of perforated postbulbar duodenal ulcer is increasing in our study with a weight of 2.19% and requires increased vigilance both diagnostically and intraoperatively.
