    <title>Maternal complications in missed miscarriage – a literature review</title>

    <summary type="text">Title: Maternal complications in missed miscarriage – a literature review

Abstract: Background and objective: Missed miscarriage is an important form of early pregnancy loss that may be associated with maternal complications. The aim of this review was to summarize the main maternal complications associated with missed miscarriage and to emphasize the importance of timely diagnosis, individualized management, and appropriate preconception care. Materials and methods: A narrative review of national and international literature was performed, focusing on maternal morbidity, complications associated with medical and surgical management, etiological factors, and reproductive implications of missed miscarriage. Results: The main maternal complications identified were hemorrhage, anemia, retained products of conception, intrauterine infection, and complications associated with uterine evacuation. Severe complications, including sepsis, hemorrhagic shock, severe anemia, and coagulation disorders, are less frequent but may occur, particularly when diagnosis or treatment is delayed. Individualized selection of expectant, medical, or surgical management is important for reducing maternal morbidity. Histopathological examination of trophoblastic and placental tissues may contribute to elucidating the mechanisms underlying pregnancy loss. Preconception assessment is also important in women with a history of missed miscarriage. Conclusions: Early diagnosis, timely management of complications, individualized treatment, and adequate preconception care are key components in the management of missed miscarriage. Further studies specifically addressing missed miscarriage are needed, as a substantial proportion of recent literature evaluates this condition together with other forms of early pregnancy loss.; Актуальность и цель: Неразвивающаяся беременность является важной формой ранней потери беременности и может сопровождаться различными материнскими осложнениями. Целью настоящего обзора было обобщение основных материнских осложнений, связанных с неразвивающейся беременностью, а также оценка значения своевременной диагностики, индивидуализированного лечения и адекватной прегравидарной подготовки. Материалы и методы: Проведен нарративный обзор национальной и международной литературы, посвященной материнской заболеваемости, осложнениям медикаментозного и хирургического лечения, этиологическим факторам и репродуктивным последствиям неразвивающейся беременности. Результаты: Основными материнскими осложнениями являются кровотечение, анемия, задержка продуктов зачатия, внутриматочная инфекция и осложнения, связанные с эвакуацией содержимого полости матки. Тяжелые осложнения, включая сепсис, геморрагический шок, выраженную анемию и нарушения свертывания крови, встречаются реже, однако могут развиваться при поздней диагностике или несвоевременном лечении. Индивидуальный выбор выжидательной, медикаментозной или хирургической тактики имеет важное значение для снижения материнской заболеваемости. Морфологическое исследование трофобластических и плацентарных тканей может способствовать уточнению механизмов внутриутробной гибели эмбриона или плода. Важное значение имеет также прегравидарная подготовка женщин с анамнезом неразвивающейся беременности. Выводы: Ранняя диагностика, своевременное лечение осложнений, индивидуальный выбор метода лечения и адекватная прегравидарная подготовка являются ключевыми компонентами ведения неразвивающейся беременности. Необходимы дальнейшие исследования, специально посвященные неразвивающейся беременности, поскольку значительная часть современной литературы рассматривает ее в составе других форм ранней потери беременности.</summary>

Abstract: В условиях реформирования системы здравоохранения и расширения возможностей выбора медицинского учреждения беременными женщинами актуальным является изучение факторов, определяющих выбор поставщика акушерских услуг. Цель исследования — выявить основные критерии выбора родовспомогательного учреждения беременными женщинами в Республике Молдова и определить факторы, имеющие значение для организации акушерской помощи. Материалы и методы. Проведено наблюдательное описательное поперечное исследование. В исследование включены 533 беременные женщины в возрасте 18–49 лет с физиологически протекающей беременностью. Сбор данных проводился в декабре 2024 – январе 2025 гг. с использованием структурированного вопросника в бумажной и онлайн-формах. Статистический анализ выполнен с использованием Microsoft Excel и SPSS; для оценки взаимосвязей применялся критерий χ², статистически значимым считалось значение p&lt;0,05. Результаты. Выбор родовспомогательного учреждения определялся сочетанием медицинских, социально-экономических и субъективных факторов. Наиболее значимыми критериями являлись профессионализм медицинского персонала — 84,8%, наличие современного медицинского оборудования — 70,8% и соблюдение санитарно-гигиенических стандартов — 64,0%. Для 54,97% женщин важными были одновременно медицинское учреждение и конкретный врач, а для 35,65% — преимущественно врач. Официальные сайты медицинских учреждений, образовательные занятия для будущих матерей и социальные сети также оказывали влияние на выбор. Выводы. Выбор поставщика акушерских услуг представляет собой комплексный процесс, в котором ведущую роль играют профессионализм медицинского персонала, техническое оснащение, безопасность и качество коммуникации. Полученные результаты могут быть использованы при совершенствовании организации акушерской помощи и разработке коммуникационных стратегий медицинских учреждений.; In the context of the reform of the health care system and the expansion of the possibilities for choosing a medical institution by pregnant women, it is relevant to study the factors that determine the choice of an obstetric service provider. The purpose of the study is to identify the main criteria for choosing a obstetric institution for pregnant women in the Republic of Moldova and to determine the factors that are important for the organization of obstetric care. Materials and methods. An observational descriptive cross-sectional study was performed. The study included 533 pregnant women aged 18-49 years with physiologically flowing pregnancies. Data collection was carried out in December 2024 – January 2025 using a structured questionnaire in paper and online forms. Statistical analysis was performed using Microsoft Excel and SPSS; χ ² test was used to assess the relationships, p &lt;0.05 was considered statistically significant. Results. The choice of obstetric institution was determined by a combination of medical, socioeconomic and subjective factors. The most significant criteria were the professionalism of medical personnel – 84.8%, the availability of modern medical equipment – 70.8% and compliance with sanitary and hygienic standards – 64.0%. For 54.97% of women, a medical institution and a specific doctor were important at the same time, and for 35.65% – mainly a doctor. Formal health facility websites, educational sessions for expectant mothers, and social media also influenced choices. Conclusions. The choice of obstetric provider is a complex process in which the professionalism of medical personnel, technical equipment, safety and quality of communication play a leading role. The results obtained can be used to improve the organization of obstetric care and the development of communication strategies for medical institutions.</summary>

    <title>Comparative analysis of vitamin K levels in women with intrahepatic cholestasis of pregnancy</title>

    <summary type="text">Title: Comparative analysis of vitamin K levels in women with intrahepatic cholestasis of pregnancy

Abstract: Background Intrahepatic cholestasis of pregnancy (ICP) is a liver condition that may impact both mother and fetus,&#xD;
including preterm birth and hemorrhage. Vitamin K, a fat-soluble vitamin essential for coagulation, may be deficient&#xD;
in ICP due to impaired bile flow, raising hemorrhage risk. The study aimed to analyze Vitamin K1, K2 MK4, and K2&#xD;
MK7 levels in pregnant women with ICP and determine associations between Vitamin K deficiency and postpartum&#xD;
hemorrhage.&#xD;
Methods This prospective cohort study included 44 pregnant women with ICP (L1) and 44 controls (L0). Serum&#xD;
Vitamin K levels, using high-performance liquid chromatography, and blood loss during delivery were assessed.&#xD;
Statistical analyses included t-tests and chi-square tests, with significance at p&lt;0.05. Study registration number&#xD;

Results Women with ICP exhibited significantly lower mean levels of Vitamin K1 (0.15±0.17 µg/L in L1 vs.&#xD;
0.29±0.30 µg/L in L0, p=0.0085) and Vitamin K2 MK7 (0.17±0.13 µg/L in L1 vs. 0.26±0.14 µg/L in L0, p=0.0024)&#xD;
compared to controls. Vitamin K1 deficiency was observed in 52.3% of the ICP group vs. 2.3% in controls. Mean blood&#xD;
loss during vaginal delivery was higher in the ICP group (351±104 mL in L1 vs. 297±87 mL in L0, p=0.0373).&#xD;
Conclusions This study suggests that ICP contributes to significant Vitamin K1 deficiency in pregnant women,&#xD;
potentially increasing postpartum hemorrhage risk. Routine Vitamin K monitoring and possible supplementation with&#xD;
vitamin K in pregnant women with ICP may be beneficial to mitigate adverse maternal outcomes. Further research is&#xD;
warranted to confirm these findings.</summary>

    <summary type="text">Title: Late fetal growth restriction: diagnostic challenges and contemporary induction strategies

Abstract: Fetal growth restriction (FGR) remains one of the major challenges of modern obstetrics. Late-onset FGR, diagnosed after 32 weeks of gestation, is the most frequent clinical form and is characterized by subtle Doppler abnormalities and preserved fetal compensatory mechanisms, making obstetrical management particularly complex. Objective. To analyze current diagnostic criteria for late fetal growth restriction and to evaluate recent evidence regarding induction of labour methods in pregnancies complicated by this pathology. Material and methods. A narrative review of recent literature was performed, focusing on modern diagnostic standards established after the Delphi consensus and comparative clinical studies evaluating mechanical and pharmacological induction methods in late FGR pregnancies. Results. The Delphi consensus improved diagnostic standardization by integrating biometric and Doppler criteria. Recent studies demonstrate that mechanical induction methods, particularly Foley balloon and extra-amniotic balloon, are associated with lower rates of uterine tachysystole, cesarean delivery for fetal distress, and neonatal intensive care admission compared with dinoprostone.  Conclusions. Late fetal growth restriction requires precise prenatal diagnosis and individualized obstetrical management. Mechanical cervical ripening methods currently appear to offer better maternal and fetal tolerance in many clinical situations.</summary>
