Title: Aspecte statistice a traumatismului izolat al fracturii de mandibulă Authors: Cazac, Cornel; Hîţu, Dumitru; Şcerbatiuc, Dumitru; Vlas, Vasile Abstract: During 23 years, the incidence of isolated mandible fractures increased up to 1,69%, which is explained by decreasing the number of hospital beds from 60 (1987) to 40 beds (2010), as well as, the decreasing number of population. Switching from the government financial assessed medicine to the state healthcare insurance program, as well as private paid medical services, brought an another factor that have been contributed to the reduction of the facial trauma. The incidence of the isolated fractures of the mandible consists 79,28% of all mandible fractures. Angular region of the mandible being held low, is involved in 70% of cases. Individual splints is frequently applied in the treatment, which is advocated by the low cost and easy manufacturing technique. Pe parcursul a 23 ani traumatismul izolat a fracturilor de mandibulă s-a mărit cu 1,69% ce se explică prin micşorarea numărul de paturi de la 60 (1987) la 40 de paturi(2010) şi micşorarea polulaţiei. Trecerea de la medicina de stat la medicina prin asigurare şi contra plată este un alt factor ce a contribuit la micşorarea traumei faciale. Fractura izolată de mandibulă ocupă 79.28% din toate traumatismele faciale. Regiunea angular a madibulei, fiind loc slab, e implicat în 70%. Fracturile de mandibulă unilaterale sau înregistrat în 36.66% şi duble în 63.33%. Atela individuală este frecvent aplicată în tratament, prin costul redus şi tehnica uşoară de confecţionare. Title: Rezultatele în timp a tratamentului osteomielitelor de maxilare Authors: Radzichevici, Mihail Abstract: At studying of the clinical and laboratory data of some patients, treated in clinic of oral and maxillofacial surgery of the CNPSDMU during 2004 – 2011 years, who were diagnosed with “chronic osteomielitis of maxillary bones” , was tried to see pathogenic factors involved in etiology of the disease. Studied patients were or are drugs consumer till present. After analyses of the clinical data of laboratory and studied literature, we can affirm that pathological bone changes are induced of the interrelation of the more action mechanisms of the consumed drugs components, non legal produced. From our clinic practical experience during the study of treatments cases of osteomyelitis we turn our attention to surgical treatment, as consequence we can achieve a satisfactory stability of fragments to which is necessary the fixation with the help of importable reconstructive plate. Positive results in the treatment of toxical osteomyelitis can be reached by the use of local insertion of embryonic marrowy cells. Studiind datele clinice şi de laborator pacienţilor trataţi în clinica de chirurgie oromaxilo-facială a CNŞPMU în perioada anilor 2004-2011, care au fost diagnosticaţi cu “osteomielită cronică a maxilarilor” s-a încercat depistarea factorilor patogenetici implicaţi în apariţia maladiei. Pacienţii studiaţi au fost sau sunt şi pînă în prezent consumatori de droguri. În urma analizei datelor clinice de laborator şi a literaturii studiate putem afirma că schimbările patologice osoase sunt induse de o interrelaţie a mai multor mecanisme de acţiune a componentelor drogului consumat, produse clandestin (perventin). Din experienţa clinicii noastre, studiind cazurile tratamentului osteomielitei, atragem atenţia asupra tratamentului chirurgical, în urma căruia am putea primi o stabilitate satisfăcătoare a fragmentelor a osului sănătos la care indispensabilă este fixarea cu plăci rigide reconstructive. Obţinem rezultate pozitive în tratamentul ostreomielitelor toxice a maxilarelor în urma administrării locale a celulelor embrionare osteomedulare. Title: Diagnosticul şi tratamentul luxaţiei anterioare bilaterale a articulaţiei temporomandibulare Authors: Rotarciuc, Valeriu; Hîţu, Dumitru; Vlas, Vasile Abstract: In 2009-2011 years period to Center National Scientific and Practical Emergency has been addressed 16 patients with anterior bilateral temporomandibular joint dislocation where have been made the clinical and paraclinical examen, as a treatment method tried to reduce the dislocation by the method of Hippocrates in all patients, the tratament was effective in 10 cases, but 6 patients did not succed and then resorted to methods of treatment are basically reducing jaw muscle tone,the method proposed by Г.Л.Блехман, Tanujan Thangarajah and accomplices. After the tratament the patients respected indications, the complications were not. În perioada anului 2009-2011 la Centrul Naţional Ştiiţifico-Practic Medicină de Urgenţă (CNŞPMU) s-au adresat 16 pacienţi cu luxaţie anterioară bilaterală a articulaţiei temporomandibulare, la care li sa efectuat examenul clinic şi paraclinic, ca metoda de tratament s-a ales reducerea luxaţiei după Hipocrate, tratamentul a fost efectiv în 10 cazuri, la cei 6 reducerea luxaţiei după Hipocrate nu s-a reuşit, din cauza tonusului marit al muşchilor masticatori. S-a recurs la procedee pentru a micşora tonusul muşchilor masticatori: procedeu propus de Г.Л. Блехман, Tanujan Thangarajah şi coautorii. Abstract: Aeschylus fracture of the zygomatic complex in the clinic of Oral and Maxillofacial Surgery from Chisinau often is fixed via osteosynthesis methods as one of the safest methods and more perfect. Immobilization with Kirschner brooches are used in exceptional cases (single cases). Endooral access and temporally immobilization of the fractured fragments permit an aesthetic recovery of the patient with trauma of the zygomatic complex. Fractura eschiloasă a complexului zigomatic în Clinica de Chirurgie OMF din Chişinău mai des se fixează prin osteosinteză fiind una din metodele cele mai sigure şi cele mai perfecte. Imobilizarea cu broşa Kirschner sunt utilizate în cazuri excepţionale(cazuri unice) rar. Accesul endobucal şi temporal de imobilizare a fragmentelor fracturate permite recuperarea estetică a bolnavului cu traumatism al complexului zigomatic.
