      <description>Title: Rolul metodelor chirurgicale în corecţia sindromului hipersplenic grav
Authors: Cazacov, Vladimir
Abstract: The clinical casuistry study consisted of 314 cirrhotic patients with SSH, with an age&#xD;
between 16 and 56 years old, admitted, monitored, diagnosed in Surgical Department Nr.2,&#xD;
hepaticobiliopancreatic surgery division. From the total of 314 selected cases that were operated&#xD;
for SSH, in 295 (94,2%) splenectomy and azygo-portal devascularisation associated with&#xD;
omentopexy was performed, of which 279 were done in classical way while 16 cases (5,4%)&#xD;
laparoscopically and one case was converted to classical. Our experience shows that the&#xD;
approach in SSH cases needs to be done carefully in organized manner, in teams specially&#xD;
trained to deal with chronic hepatic pathology patients.&#xD;

Studiul cazuisticii clinice a fost alcătuit dintr-un număr de 314 de pacienţi cirotici cu HSS,&#xD;
cu vîrsta între 16 şi 56 de ani internaţi, monitorizaţi, diagnosticaţi şi trataţi în Clinica 2 Chirurgie,&#xD;
compartimentul de Chirurgie Hepatobiliopancreatică. Din numărul total de cazuri selectate şi&#xD;
operate pentru HSS, la 295 (94,2%) au fost practicate splenectomia cu devascularizare azygoportală&#xD;
asociată cu omentopexie dintre care la 279 pacienţi s-a intervenit prin metode clasice, iar&#xD;
la 16 pacienţi (5,4%) prin metoda videolaparoscopică, un caz convertit la metoda clasică.&#xD;
Experienţa acumulată arată că abordarea cazurilor cu splenopatie portală atent selectate trebuie&#xD;
efectuată în echipă organizată, special antrenată în managementul pacienţilor cu tară hepatică.

Authors: Şcerbina, Romeo; Ghidirim, Gheorghe; Lescov, Vitalie; Manea, Vladimir; Boghean, Gheorghe
Abstract: The authors present a short literature review and two clinical cases – the comlications of&#xD;
pulmonary tuberculosis treatement : the peptic perforated duodenal ulcer and hemoragic ulcer. In&#xD;
both cases the antiulcers treatment required for this situation was ignored. As a conclusion the&#xD;
authors recommend antiulcers treatement and endoscopic dynamic examinations to all patients&#xD;
with tuberculosis.&#xD;

Autorii prezintă un scurt reviu al literaturii şi două cazuri clinice – complicaţii ale&#xD;
tratamentului tuberculozei pulmonare: hemoragie ulceroasă şi ulcer perforat. În ambele cazuri a&#xD;
fost ignorat tratamentul antiulceros necesar în aceste situaţii. În concluzie se recomandă&#xD;
tratamentul antiulceros şi supravegherea endoscopică dinamică a tuturor pacienţilor cu&#xD;
tuberculoză.

      <title>Asimetria fluxului spleno-portal la bolnavul cirotic</title>
      <link>http://repository.usmf.md:80/handle/20.500.12710/4557</link>
      <description>Title: Asimetria fluxului spleno-portal la bolnavul cirotic
Authors: Anghelici, Gheorghe; Moraru, Viorel; Gaidău, Margarita; Ţarălungă, Veronica
Abstract: The patients Child class "A" denotes a proportional distribution volume of the portal&#xD;
intrahepatic hemocirculation (comparable with healthy subjects) with a slight upward trend while&#xD;
patients in group Child "B" and "C" evolving expressive distortion of intrahepatic portal&#xD;
hemodynamics with a significant deprivation as liver lobe.Reduced hepatic functional reserve is&#xD;
associated with a progressive hemodynamic redistribution in the territory and establishing a pool&#xD;
portal splenic venous hiperdinamice movement, recorded is a positive correlation between&#xD;
increased venous congestion in the splenic vein territory and Child score. It was found a clear&#xD;
tendency to increase intake flow in the formation of splenic venous outflow on the left portal&#xD;
branch and a direct connection between the report SBF/ PBFand category Child-Pugh,&#xD;
correlation becomes more expressive on reducing the extent of liver functional reserves.&#xD;
&#xD;
La pacienţii categoriei Child ”A” se denotă o distribuire volumetrică proporţională a&#xD;
hemocirculaţiei portale intrahepatice (comparabilă cu subiecţii sănătoşi) cu o tendinţă de creştere&#xD;
usoară a circulaţiei sanghuine pe lobul hepatic stîng, pe cînd în grupa pacienţilor Child ”B” şi&#xD;
„C” evoluează o denaturare expresivă a hemodinamicii portale intrahepatice cu o deprivare&#xD;
hemocirculatorie semnificativă a lobului hepatic drept. Reducerea rezervelor hepatice&#xD;
funcţionale se asociază cu o redistribuire progresivă a hemocirculaţiei în teritoriul bazinului&#xD;
portal şi instituirea unei circulaţii splenice venoase hiperdinamice, consemnîndu-se o corelaţie&#xD;
pozitivă între creşterea stazei venoase în teritoriul venei splenice şi scorul Child. A fost&#xD;
constatată o tendinţă evidentă de creştere a aportului afluxului venos splenic în constituirea&#xD;
fluxului pe ramura portală stînga şi o conexiune directă între raportul SBF/PBF şi categoria&#xD;
Child - Pugh, corelaţie ce devine mai expresivă pe măsura reducerii rezervelor hepatice.

      <title>Stem cells and hepatocytes in therapy of liver diseases</title>
      <link>http://repository.usmf.md:80/handle/20.500.12710/4559</link>
      <description>Title: Stem cells and hepatocytes in therapy of liver diseases
Authors: Mocan, Elena; Lîsîi, Corneliu; Boaghie, Natalia; Revencu, Tatiana; Nacu, Viorel
Abstract: În prezent, există un interes crescut a utilizării celulelor stem în terapia diferitor boli.&#xD;
Celulele stem au capacitatea de a se devide pentru o perioadă indefinită de timp, de a se autoreînoi&#xD;
şi de a da naştere mai multor tipuri de celule. Terapia celulară poate fi definită ca&#xD;
„utilizarea celulelor în restabilirea, menţinerea şi îmbunătăţirea funcţiilor ţesuturilor şi&#xD;
organelor”. Acest articol de sinteză este adresat hepatocitelor, celulelor stem hepatice, inclusiv&#xD;
originii lor, rolului acestora în regenerarea ficatului şi dezvoltarea fibrozei, şi folosirea lor&#xD;
posibilă în tratarea bolii de ficat. Cele mai multe tulburări hepatice rezultă din disfuncţia&#xD;
hepatocitelor, aici acordîndu-se un mare interes transplantului de hepatocite izolate. În închiere,&#xD;
hepatocitele izolate şi derivate trebuie să demonstreze activitatea normală fiziologică, anume în&#xD;
funcţia de detoxificare şi metabolismul specific, ce se determină prin expresia genelor a&#xD;
proteinelor prezente în hepatocitele adulte.&#xD;
&#xD;
At present, there is growing interest in the therapeutic use of stem cells. A stem cell has the&#xD;
ability to divide for indefinite periods of time, to self-renew and to give rise to many different&#xD;
cell types. Cell therapy can be defined as «the use of living cells to restore, maintain or enhance&#xD;
the function of tissues and organs». This review will address on hepatocytes, hepatic stem cells,&#xD;
including their origin, their role in liver regeneration and fibrosis, and their possible use in the&#xD;
treatment of liver disease. As most liver disorders result from hepatocyte dysfunction, there has&#xD;
been great interest in transplantation of isolated hepatocytes. In summary, isolated and derived&#xD;
hepatocytes should demonstrate drug metabolism and detoxification activity by both gene&#xD;
expression and function, and they should express the hepatic transport proteins and transcription&#xD;
factors present in mature hepatocytes.
