Since 2005, the Nicolae Testemitsanu State University of Medicine and Pharmacy has become the co-founder of this journal. Abstract: Delirium is a disturbance of consciousness, attention, cognition and perception which develops over a short period of time and tends to fluctuate during the course of the day. Delirium occurs in 60-80% of intensive care patients and is commonly missed. Delirium is an independent predictor of prolonged hospital and intensive care unit stay, reintubation, cost of care, long-term cognitive impairment and mortality. The article discusses subtypes of delirium (hyperactive, hypoactive and mixed), risk factors and pathogenesis. Validated test for detecting delirium are the intensive care delirium screening checklist (ICDSC) and the confusion assessment method for ICU (CAM-ICU). The treatment concentrates on risk factors modification, and the drug of choice in the treatment of delirium is haloperidol, although atypical anti-psychotics (olanzapine, risperidone, ziprasidone) are as effective as haloperidol with fewer side effects.; Делирий характеризуется расстройством сознания, внимания, когнитивных функций и восприятия, флюктуирующие в течении суток. В отделении интенсивной терапии 60-80% пациентов переживают эпизоды делирия. При развитии делирия продолжается длительность пребывания в отделении интенсивной терапии и стационаре, частота реинтубации, стоимость лечения, когнитивные расстройства и летальность. В статье рассматриваются клинические варианты делирия (гипоактивный, гиперактивный, смешанный), факторы риска, а также патогенез этого состояния. В повседневной практике для диагностики делирия применяется скрининговая шкала и метод диагностики спутанности, разработанные для пациентов, находящихся в отделении интенсивной терапии. Лечение делирия осуществляется препаратом галоперидол, хотя и атипичные антипсихотики (оланзапин, рисперидон, зипрасидон) эффективны в лечении этого состояния, а побочные эффекты минимальны.
