Abstract: Rezumat. Anii Potențiali de Viață Pierduți (APVP) reprezintă un indicator esențial de sănătate publică, utilizat pentru a cuantifica impactul mortalității premature și a identifica domeniile prioritare de intervenție. Scopul acestui studiu transversal este analiza tendințelor APVP și a principalelor cauze ale deceselor premature în Republica Moldova în perioada 2014–2023, cu accent pe disparitățile regionale, de sex și mediu de reședință (urban/rural). A fost utilizată o bază de date depersonalizată a certificatelor medicale de deces pentru perioada 2014–2023, furnizată de Agenția Națională pentru Sănătate Publică. Au fost incluse 177 273 de înregistrări de deces ale persoanelor cu vârsta între 15 și 69 de ani. APVP a fost calculat ca valoare absolută și ca medie APVP per deces. Între 2014 și 2023, numărul total de APVP a scăzut cu peste 30%. În 2023, APVP a fost de 125 511 ani la bărbați și 49 469 ani la femei. Aproximativ 65% dintre decesele premature au avut loc în mediul rural, variind de la 18% în Chișinău la 34% în regiunea Centru. La bărbați, principalele cauze au fost bolile cardiovasculare (30%), cauzele externe (19%), neoplasmele (17%) și bolile aparatului digestiv (15%). Cele mai mari valori medii APVP per deces au fost pentru cauzele externe (21 de ani) și bolile infecțioase (19,7 ani), iar cele mai mici pentru bolile cardiovasculare și neoplasme (9,5 ani). Municipiul Chișinău a înregistrat constant cele mai scăzute niveluri, în timp ce Sudul și Centrul au prezentat cele mai mari valori absolute, dar și cel mai important progres relativ. În Nord, declinul a fost mai modest.; Summary. Years of Potential Life Lost (YPLL) is a key public health indicator used to quantify the impact of premature mortality and to identify priority areas for intervention. The aim of this crosssectional study is to analyse trends in PYLL and the main causes of premature deaths in the Republic of Moldova during 2014–2023, with a focus on regional, sex, and urban–rural disparities. We used a depersonalized database of medical death certificates for 2014–2023, provided by the National Agency for Public Health. A total of 177 273 death records for individuals aged 15–69 were included. YPLL was calculated both as absolute values and as mean YPLL per death. Between 2014 and 2023, total YPLL decreased by over 30%. In 2023, YPLL amounted to 125 511 years for males and 49 469 years for females. About 65% of premature deaths occurred in rural areas, varying from 18% in Chișinău to 34% in the Centre region. Among males, the leading causes were cardiovascular diseases (30%), external causes (19%), neoplasms (17%), and digestive diseases (15%). The highest mean YPLL per death was observed for external causes (21 years) and infectious diseases (19.7 years), while cardiovascular diseases and neoplasms had the lowest (9.5 years). At the regional level, Chisinau consistently reported the lowest PYLL, while the South and Centre recorded the highest absolute values, as well as the most significant relative progress. Потерянные потенциальные годы жизни (ППГЖ) — важный показатель общественного здоровья, используемый для оценки бремени преждевременной смертности и определения приоритетных направлений вмешательств. Цель данного поперечного исследования — анализ тенденций ППГЖ и основных причин преждевременной смертности в Республике Молдова в 2014–2023 гг., с акцентом на региональные, гендерные и город/село различия. Использована деперсонализированная база данных медицинских свидетельств о смерти за 2014–2023 гг., предоставленная Национальным агентством общественного здоровья. В анализ включено 177 273 записей о смерти лиц в возрасте 15–70 лет. ППГЖ рассчитаны как в абсолютных значениях, так и как среднее число ППГЖ на один случай смерти. В период 2014–2023 гг. общее количество ППГЖ сократилось более чем на 30%. В 2023 г. было потеряно 125 511 лет у мужчин и 49 469 лет у женщин. Около 65% преждевременных смертей пришлись на сельскую местность, с региональной вариацией от 18% в Кишиневе до 34% в Центре. У мужчин основными причинами ППГЖ были сердечно-сосудистые заболевания (30%), внешние причины (19%), новообразования (17%) и болезни органов пищеварения (15%). Наибольшее среднее число ППГЖ на один случай смерти наблюдалось при внешних причинах (21 год) и инфекционных заболеваниях (19,7 года); наименьшее — при сердечно-сосудистых заболеваниях и новообразованиях (9,5 года). На региональном уровне Кишинёв стабильно фиксировал самые низкие показатели, в то время как Юг и Центр демонстрировали наибольшие абсолютные значения, но также и наиболее значительный относительный прогресс. Abstract: Thesis volume. The thesis is presented on 129 pages of basic text and includes an introduction, 4 chapters, general conclusions and practical recommendations. The bibliography includes 293 sources. The work is completed with annexes, a list of publications and participation in scientific forums, a statement on assuming personal responsibility, annotation in Romanian/English and the author's CV. The basic text is illustrated with 24 tables, 2 figures and 13 formulas. Field of study: 323.01 – Dentistry Aim of the thesis: characterization of the clinical, functional, imaging-structural and kinematic expression in patients with temporomandibular disorders and condylar malposition, in order to identify specific characteristics. Study objectives: 1. Evaluation of the clinical picture and functionality of patients with TMD and condylar malposition based on the DC/TMD protocol, regarding the distribution of orofacial pain, the degree of functional limitation, the intensity and chronicity of pain and indicators of psycho-emotional impairment. 2. Qualitative and quantitative imaging examination of the morphology of the condyle, glenoid fossa and articular eminence, as well as the distribution of sagittal and coronal joint spaces in patients with TMD and condylar malposition. 3. Highlighting the cephalometric particularities of the investigated group, based on the distribution of skeletal classes and analyses by authors (Downs-Graber, Sato, Slavicek, Kim). 4. Threedimensional assessment of mandibular kinematics in opening, protrusion and mediotrusion by condylography, using both standard indices (X, Y, Z displacements, SCI, TCI) and derived parameters, to assess the organization of condylar movements. 5. Integrated analysis of clinical, imaging and functional data in patients with temporomandibular disorders and condylar malpositions. Scientific novelty and originality: An element of novelty consists in the integrated evaluation, on the same group of patients with TMD and condylar malpositions, of the condyle–fossa relationship by CBCT, condylar kinematics by 3D condylography, and the clinical, functional and psychobehavioral profile by DC/TMD and Axis II instruments. Morphometric variations of the glenoid fossa, articular eminence, condyle and joint spaces were identified, with deviations of linear and composite indices, including DIkeda, from conventionally centric reference values, revealing the multidimensional expression of condylar malposition in the sagittal, vertical and coronal planes. The originality of the study is supported by the definition and application of derived condylographic indices of amplitude, orientation and curvature, calculated separately for the balancing condyle and the working-side condyle. Stable groups of sagittal and transverse variables were identified, including the SCI–SagAng and TCI–TransAng pairs, as well as kinematic differences between the balancing and working-side condyles, the latter showing greater numerical variability during movements with reduced excursion. The study also reveals the association of condylar malposition with high pain and functional burden, mobility limitations, extensive muscle tenderness and increased GAD-7/PHQ-9 scores, outlining a complex clinical-imaging and psychobehavioral profile. Applicative value. By configuring a unified protocol combining DC/TMD Axis I and Axis II, additional clinical indices, CBCT assessment of the condyle–fossa relationship and 3D condylography, the study provides a clinically applicable framework for diagnosing and monitoring patients with TMD and condylar malposition. The described parameters can be integrated into standardized examination forms and followup protocols, facilitating longitudinal comparison of cases and objective assessment of treatment response. CBCT and condylographic indices may serve as references for imaging and functional interpretation, including in cases of condylar repositioning guided by splints or other devices. Data on psychological burden and functional limitations may guide referral to specialists in orofacial pain, psychology or rehabilitation medicine, supporting interdisciplinary management of these patients. Implementation of results: The results were consolidated through 5 innovator certificates (methods/indicators for assessing condylar position and kinematics) and implemented in the clinical activity of the “OrtoClinic” SRL Dental Clinic and the Department of Interdisciplinary Dentistry and Oral Rehabilitation of Nicolae Testemitanu State University of Medicine and Pharmacy.</description>
