Although there are definite criteria for initiating nonoperative management of liver and spleen blunt lesions, the specific criteria that would determine which patient is prone to develop hemodynamic instability after an abdominal injury are not yet established. Failure of nonoperative treatment usually leads to surgery for patients that continue to require transfusions or have altered vital signs and may have an adverse result. That is why the search for both clinical and imaging criteria of severity that would ensure successful therapeutic option continues.
