Opţiuni terapeutice actuale în sindromul hipertensiv portal şi hipersplenism

Abstract

Tratamentul hipersplenismului sever secundar hipertensiunii portale cirogene ridică o serie de probleme practice. Înţelegerea etiopatogeniei, a diagnosticului şi a tratamentului hipertensiunii portale şi hipersplenismului secundar s-a schimbat drastic în ultimii 30 de ani, dar acesta continuă să fie un factor important care influienţează morbiditatea şi mortalitatea. Alegerea momentului operator este un subiect de dezbatere; în permanenţă, trebuie adaptată strategia terapeutică în funcţie de elementele clinice şi/sau biologice care apar în cursul evoluţiei bolii. Aprofundarea mecanismelor responsabile de tulburările hemodinamicii portale şi adaptarea unei atitudini terapeutice adecvate va ameliora prognosticul grav al acestei categorii de bolnavi.

In this article is presented the news related to treatment and diagnostic approaches of the portal hypersplenism that nowadays, represents an actual problem of health. Understanding of the pathogenesis, diagnosis, and treatment of portal hypertension has drastically changed during the last 30 years, but it continues to be a frequent cause of morbidity and mortality. This fact is due to the distressful increasing of its prevalence. A better prognosis for hypersplenism is possible only by elucidating the mechanism which produced the dysfunction hemodynamic portal and by promoting an aggressive therapeutic approach.
