Rezultatele chestionarului „SF-LDQOL” de evaluare a calităţii vieţii pacientului cirotic chirurgical tratat

Abstract

Introducere: Evaluarea şi îmbunătăţirea instrumentelor existente de apreciere a calităţii vieţii (CV), precum şi punerea acestora în aplicarea practică, rămâne un subiect important, mai ales în cazul terapiei bolilor hepatice cronice. Scopul: Lucrarea de faţă îşi propune să analizeze aspectele comparative privind calitatea vieţii pacienţilor cirotici operaţi. Material şi metode: În studiu au fost incluşi 68 pacienţi cirotici operaţi având cel puţin 12 luni de supraveghere postoperatorie şi care au completat chestionarul „SF-LDQOL” alcătuit din 9 domenii urmărind să determine nivelul CV acestora. Repartiţia cazurilor pe 3 loturi de studiu s-a efectuat în funcţie de tratamentul aplicat, lotul martor a inclus 20 subiecţi sănătoşi. Rezultate: După calcularea parametrilor ce definesc CV pacienţilor încadraţi în studiu, am obţinut o afectare severă cu deviaţia standard +50,16 a stării de sănătate fizică (PCS) şi +45,12 a stării de sănătate mentală (MCS). Am constatat că instituirea tratamentului multimodal (medicamentos, endoscopic şi chirurgical) are de regulă un impact major asupra domeniilor CV investigate cu chestionarul „SF-LDQOL”, dar variabil în funcţie de gestul chirurgical aplicat, gradul mai înalt fiind atribuit pacienţilor post-transplant hepatic. Concluzii: Experienţa acumulată oferă sprijin pentru fiabilitatea şi validitatea chestionarului „SF-LDQOL”, instrument specific, util pentru aprecierea CV pacienţilor cu suferinţă hepatică; planificarea şi evaluarea performanţei serviciilor medico-chirurgicale.

Assessment and improvement of existing tools for assessing the quality of life (QoL) and putting them in thepractical application remains an important issue, especially in chronic liver disease therapy. Aim To analyze the comparative aspects of QoL in operated cirrhotic patients.Material and methods_ The study included 68 operated cirrhotic patients with at least 12 months of postoperative follow-up andwho filled in the questionnaire „SF-LDQOL” consists of 9 areas aimed to determine the level of their QoL. Distribution of casesamong three study groups was done according to the applied treatment. The control group included 20 healthy subjects. Results After calculating the parameters defining QoL of patients enrolled in the study, we obtained a standard deviation of+50.16 indicating severe damage to physical health (PCS) and +45.12 for a mental health condition (MCS). We found that the imposition of multimodal treatment (medication, endoscopic and surgical) usually has a major impact on the areas investigated by the „SF-LDQOL”, but varies with the applied surgical method, the highest level being awarded patients after liver transplant.Conclusions_ The experience provides support for reliability and validity of the questionnaire „SF-LDQOL” – useful tool for the assessment of QoL in patients with hepatic pathology, as well as for planning and performance of evaluation of surgical services.
