Errors at the determination and registration of the intermaxillary relationship

Abstract

Background. The determination and registration of the intermaxillary relationship is an intermediate clinical stage, which ensures the creation of the morphofunctional balance between the prosthetic part and the components of the stomatognathic system. The possible errors, committed at this stage, will compromise the correct realization of the occlusion relations and the morphofunctional conservation of the stomatognathic system for a long time. Objective of the study. Analysis of possible errors at the stage of determination and registration of intermaxillary relationships, evaluation of clinical aspects, diagnosis and prevention of possible complications. Material and Methods. 73 (44m., 29w.) patients, aged between 22 and 74 years, with various forms of uni or bimaxillary partial edentations were selected, examined and treated prosthetically. All patients were divided into two groups: a) patients, in whom PIM was determined under the conditions of the oral cavity, b) patients, in whom the position of CR was determined, using templates with occlusion border. Results. The clinical-technical algorithm for determining and recording the intermaxillary relationship was determined by the particularities of the clinical case, in strict compliance with the provisions and general classical principles for this work. The results of clinical and paraclinical examinations at the trial stage of the construction of future prostheses in the oral cavity, immediately after treatment and at a distance, 3-6 months, 1-3 years, showed that strict adherence to the clinical-technical algorithm for determining and recording intermaxillary relations in the case of the edentulous partially proposed by us ensured in the end the integration of the prothetic parts and the preservation of the optimal functional state of the stomatognathic system. Conclusion. It is important to determine the characteristic of the intermaxillary relationship, which is to be determined and recorded. The observance of the algorithm for determining the intermaxillary relationship with the use of the template with the occlusion rims ensures the correctness of the given workmanship.

Introducere. Determinarea și înregistrarea relației intermaxilare reprezintă o etapă clinică intermediară, care asigură crearea echilibrului morfofuncțional dintre piesa protetică și componentele sistemului stomatognat. Erorile posibile, comise la această etapă, vor compromite realizarea corectă a relațiilor de ocluzie și conservarea morofofuncțională a sistemului stomatognat pentru un timp îndelungat. Scopul lucrării. Analiza eventualelor erori la etapa de determinare și înregistrare a relațiilor intermaxilare, evaluarea aspectelor clinice, de diagnostic și de prevenire a complicațiilor posibile. Material și Metode. Au fost selectaţi, examinaţi şi trataţi protetic 73 (44 b., 29 f.) pacienţi, cu vârsta cuprinsă între 22 şi 74 de ani, cu diferite forme de edentaţii parţiale uni sau bimaxilare. Toți pacienții au fost distribuiți în două grupe: a) pacienții, la care a fost determinată PIM în condițiile cavității bucale; b) pacienții, la care a fost determinată poziția de RC, cu utilizarea șabloanelor cu bordură. Rezultate. Algoritmul clinico-tehnic de determinare și înregistrare a relației intermaxilare a fost determinat de particularitățile cazului clinic, cu respectarea strictă a prevederilor și principiilor generale clasice pentru această manoperă. Rezultatele examenelor clinic şi paraclinic la etapa de probă a construcțiilor viitoarelor proteze în cavitatea bucală, imediat după tratament şi la distanţă, 3 - 6 luni, 1-3 ani, au demonstrat că respectarea cu stricteţe a algoritmului clinico-tehnic de determinare și înregistrare a relațiilor intermaxilare în cazul edentatului parțial propus de noi a asigurat în final integrarea pieselor proteice şi conservarea stării funcţionale optime a sistemului stomatognat. Concluzii. Este important de a determina caracteristica relației intermaxilare, care urmează a fi determinată și înregistrată. Respectarea algoritmului de determinare a relației intermaxilare cu utilizarea șablonului cu bordură de ocluzie asigură corectitudinea realizării manoperei date.
