Particularități ale calității vieții copiilor cu maladii oncohematologice

Abstract

Rezumat Cancerul este a doua cauză principală de deces la nivel global, reprezentând un număr estimat de 9,6 milioane de decese, sau unul din șase decese. Anual, peste 300.000 de copii sunt diagnosticați cu cancer, din care 100.000 decedează. În 2019, în Republica Moldova, incidența tumorilor maligne la copii a fost de 16,8 la 100.000 locuitori. În perioada decembrie 2019 – februarie 2020, a fost realizat un studiu transversal descriptiv prin intermediul unui chestionar (PedsQL 3.0, modulul oncologic). Lotul de cercetare a inclus 45 de copii cu vârste între 5 și 18 ani și 30 de familii ale acestora, cei din mediul de trai rural constituind 87% și din cel urban – 13% din cazuri; vârstă medie a copiilor = 11,58±0,58 ani. Scorurile calității vieții copiilor oncohematologici s-au repartizat astfel: durere și suferințe 62,30% (95% CI: 61,45-63,15); grețuri 63,03% (95% CI: 62,37-63,69); anxietatea legată de proceduri 58,06% (95% CI: 57,15-59,11); anxietatea legată de tratament 74,31% (95% CI: 73,52-75,10); îngrijorări 56,44% (95% CI: 55,49-63,15); probleme cognitive 62,78% (95% CI: 62,08-63,48), percepția fi zicului 52,09% (95% CI: 50,95-53,23); comunicarea 60,38% (95% CI: 59,40-61,36); scorul integral 61,11% (95% CI: 60,22- 61,99). Rezultatele studiului au constatat că ”autoevaluarea copilului” și ”evaluarea părinților” a calității vieții permit completarea informației tradiționale clinice și paraclinice de examinare. PedsQL3.0 asigură obținerea unei informații unice privind starea de sănătate, problemele psihoemoționale și cele sociale. Tendința de subestimare a calității vieții din partea părinților argumentează importanța de autoevaluare din partea copilului.

Summary Cancer is the second leading cause of the globally death, accounting the estimated deaths of 9,6 million, or one in six. Annually more than 300.000 children are diagnosed with cancer, of which 100.000 die. In 2019, in the Republic of Moldova the incidence of malignant tumors at children was 16,8 per 100 thousand inhabitants. During December 2019 – February 2020, the descriptive cross-sectional study was performed using the questionnaire (PedsQL 3.0 – cancer module). The research sample included 45 children aged 5-18 years and 30 their family; living environment: rural 87% and urban 13% of cases; with an average age of children of 11,58±0,58 years. The scores of the quality of life of onco-hematological children were distributed: Pain and Hurt 62,30% (95% CI: 61,45-63,15); Nausea 63,03% (95% CI: 62,37-63,69); Procedural anxiety 58,06% (95% CI: 57,15-59,11); Treatment anxiety 74,31% (95% CI: 73,52-75,10); Worry 56,44% (95% CI: 55,49-63,15); Cognitive problems 62,78% (95% CI: 62,08-63,48), Perceived physical appearance 52,09% (95% CI: 50,95-53,23); Communication 60,38% (95% CI: 59,40-61,36); Integral score 61,1% (95% CI: 60,22-61,99). The study results, which analyzed the data on “child self-report” and “parent proxy-report”, concluded that the quality of life allows the completion of traditional clinical and paraclinical examination information. PedsQL 3.0 – allows to obtain unique information on health status, psycho-emotional and social problems. The tendency to underestimate the quality of life because of parents’ carelessness for the importance of self-assessment on the part of the child.

Резюме Рак является второй ведущей причиной смерти во всем мире, на него пришлось около 9,6 миллиона смертей, или одна из шести смертей. Ежегодно у более 300.000 детей диагностируется рак, из которых более 100.000 умирают. В 2019 году в Республике Молдова заболеваемость злокачественными новообразованиями у детей составила 16,8 на 100.000 жителей. В течение декабря 2019 г. – февраля 2020 г. было проведено описательное поперечное исследование с помощью стандартизированного специализированного опросника (PedsQL 3.0 – онкологический модуль). Выборка исследования включила 45 детей в возрасте 5-18 лет и 30 родителей, проживающих в сельской местности 87% и городской 13%; средний возраст детей = 11,58±0,58 лет. Индексы качества жизни онко-гематологических детей были: боль и страдания 62,30% (95% ДИ: 61,45-63,15); тошнота 63,03% (95% ДИ: 62,37-63,69); боязнь процедур 58,06% (95% ДИ: 57,15-59,11); боязнь лечения 74,31% (95% ДИ: 73,52-75,10); беспокойство 56,44% (95% ДИ: 55,49-63,15), когнитивные проблемы 62,78% (95% ДИ: 62,08-63,48), восприятие внешности 52,09% (95% ДИ: 50,95-53,23); общение 60,3% (95% ДИ: 59,40-61,36); интегральная оценка 61,11% (95% ДИ: 60,22-61,99). Результаты исследования показали, что «самооценка» и «оценка родителями» качества жизни позволяют достоверно дополнить традиционные клинические и пара-клинические обследования. Опросник PedsQL 3.0 позволяет получить уникальную информацию о состоянии здоровья, психоэмоциональных и социальных проблемах. Тенденция заниженной оценки качества жизни со стороны родителей, свидетельствует о важности самооценки со стороны ребенка.
