The psychotherapeutic aspect of psychic trauma in epilepsy

Abstract

Background. The psychotherapeutic understanding of the patient suffering from epilepsy is situated in a bio-psycho-familial and social context.Whether it's the first, the second, or the next crises, they point out that there is an intrapsychic trauma. Objective of the study. The synthesis of the literature dedicated to psychotraumatic factors in epileptogenesis as well as the efficiency of non-medicamentous methods. Material and Methods. To achieve the proposed goal we performed the analysis of the literature review between 2010-2020, using 47 bibliographic sources, including the Medical Scientific Library of the USMF "Nicolae Testemițanu", data from the electronic libraries PubMed, MedScape and Hinari. Results. The management of epilepsy can be reviewed in a multidimensional way, medical, psychotherapeutic, familial and social.In this regard, we are talking about the place given to the patient suffering from epilepsy.From a psychopathological point of view, there is a system of interactions between crisis and personality based on confusional anxieties, related to ambiguity and the feeling of crisis for the subject.System of interactions between epilepsy, the patient and the environment contributes to the creation of the framework that will provide opportunities to help the patient.Psychotherapy sessions allow the patient to reintroduce the crisis in its history. Conclusion. The description of psychological experiences that are associated with the evolution of epilepsy symptoms, will allow us to form an idea about the influence of psychic trauma on the clinic and the dynamics of epilepsy, which could help identify an approach to the patient adaptation.

Introducere. Înțelegerea psihoterapeutică a pacientului, care suferă de epilepsie este situată într-un context bio-psiho-familial și social. Indiferent dacă este prima, a doua criză sau următoarele crize, ele evidențiază că exista un traumatism intrapsihic. Scopul lucrării. Sinteza literaturii dedicate factorilor psihotraumatizanți în epileptogeneză, precum și eficiența metodelor non-medicamentoase. Material și Metode. Pentru realizarea scopului propus am efectuat analiza reviului literaturii între anii 2010-2020, utilizând 47 de surse bibliografice, dintre care ale Bibliotecii Ştiinţifice Medicale ale USMF „Nicolae Testemiţanu”, date ale bibliotecilor electronice PubMed, MedScape şi Hinari. Rezultate. Managementul epilepsiei poate fi revizuit într-un mod multidimensional, medical, psihoterapeutic, familial și social. În acest sens, vorbim despre locul acordat pacientului care suferă de epilepsie.Din punct de vedere psihopatologic, există un sistem de interacțiuni între criză și personalitate bazat pe anxietățile confuzionale, legate de ambiguitate și de sentimentul de criză pentru subiect. Sistem de interacțiuni între epilepsie, pacientul și mediul înconjurator, contribuie la crearea cadrului, care va oferi oportunități pentru a ajuta pacientul. Ședințele de psihoterapie permit pacientului reintroducerea crizei în istoria lui. Concluzii. Descrierea experiențelor psihologice, care sunt asociate cu evoluția simptomelor epilepsiei, va permite să formăm o idee despre influența traumelor psihice asupra clinicii și asupra dinamicii epilepsiei, ce ar putea ajuta la identificarea unei abordări a adaptării pacientului.
