The management of the postoperative complications Roux-en-Y gastric bypass

Abstract

Background. Obesity is in a spectaculous increase over the last decades. Bariatric surgery is the only option to significant and sustained weight loss and to improvement of comorbidities and mortality decrease. Efficiency of bariatric surgery is well-studied, but its complications have been underestimated. Objective of the study. Elaboration of both diagnostic and treatment algorithm for early postoperative complications after Roux-en-Y gastric bypass. Material and Methods. The clinical trial is based on the evaluation of early postoperative results of 433 patients who supported Roux-en-Y gastric bypass during 2009-2020. Weight varied from 80 to 245 (121,8 ± 26,3) kg, age was of 18-68 (40,5±10,4) years, sex ratio was 3,5/1, with the prevalence of women. Results. Early postoperative complications were recorded in 44 (10,1%) cases. The complications mostly consisted of bleeding of the transit excluded stomach – 17 (38,6%) cases, wound suppuration – 11 (25%) cases and acute gastric dilatation – 6 (13,6%) cases. All patients (n=6) with acute gastric dilatations were reoperated via parietal gastrostomy. Conclusion. Postoperative complications after Roux-en-Y gastric bypass are dominated by bleeding of the transit excluded stomach. Gastric acute dilatations is a medical emergency, the solution being parietal gastrostomy.

Introducere. Obezitatea este în creștere spectaculoasă în ultimele decenii. Chirurgia bariatrică este singura opțiune care duce la pierderea marcată și susținută a greutății corporale. Eficiența chirurgiei bariatrice a fost bine studiată, dar complicațiile acesteia au fost subestimate. Scopul lucrării. Elaborarea algoritmului de diagnostic și tratament în complicațiile postoperatorii precoce survenite după Roux-en- Y gastric bypass. Material și Metode. Studiul clinic este bazat pe evaluarea rezultatelor postoperatorii precoce a 433 pacienți care au fost supuși Roux-en-Y gastric bypass pe perioada 2009-2020. Masa corporală a variat între 80 și 245 (121,8 ± 26,3) kg, vârsta medie - 40,5±10,4 ani, raport femei/bărbați de 3,5/1. Rezultate. Complicații postoperatorii precoce s-au înregistrat în 44 (10,1%) cazuri. Complicațiile au fost dominate de hemoragii din stomacul exclus din tranzit – 17 (38,6%) cazuri, supurația plăgii – 11 (25%) cazuri și dilatarea acută gastrică, instalată în 6 (13,6%) cazuri. Toți pacienții (n=6) la care a survenit dilatare acută gastrică au fost reoperați prin montarea gastrostomei parietale. Concluzii. Complicațiile postoperatorii precoce după Roux-en-Y gastric sunt dominate de hemoragiile din stomacul exclus din tranzit. Dilatarea acută gastrică este o urgență chirurgicală, soluția fiind aplicarea gastrostomei parietale.
