Activitatea instituţiei de nivel terţiar în domeniul ocrotirii sănătăţii mamei şi copilului şi perspectivele de dezvoltare continuă

Abstract

Summary. In this article is emphasized the fact that, depending on maternal and child risk, observation over them is carried out on I (district), II (municipal) and III (Republican) levels. The Public Medico-Sanitary Institution activity of The Research Institute for Mother and Child Health Care is the only institution of the III level, with a capacity of 1005 beds, including 275 obstetric care, 450 children’s somatic and 280 beds for children with a surgical profile. The article analyzes the main indicators of the Institute after the introduction of compulsory health insurance (2004-2009). It must be stressed that the volume of the given specialized medical care in this facility increases every year, from 27 508 patients in 2004 to 32 055 in 2009. Bed occupancy average activity per year is of 260- 280 days, bed occupancy average activity per year is of 8,0-8,9 days, the mortality rate - 0,5%, the proportion of patients from rural areas 62.4%. During those years, the financing of the Institute has significantly increased from 32.1 million lei in 2004 to 102.1 million lei, envisaged for 2010. Despite this increase, the funding is insufficient to establish a Level III specialized medical care. Further along will be shown the dynamics and the volume of medical services provided to patients and the dynamic of changes in the costs of treating one case. In conclusion, the author points to the existing problems in the management of the III level institutions, makes suggestions for resolving them, and recommendations as to further development.

Резюме. В данной статье подчеркивается, что в зависимости от материнского и детского риска наблюдение за ними осуществляется на Ι (районном), ΙΙ (муниципальном) и ΙΙΙ (республиканском) уровнях. Публичное медико-санитарное учреждение Научно-Исследовательский Институт Охраны Здоровья Матери и Ребенка является единственным учреждением ΙΙΙ уровня, мощностью 1005 коек, в том числе 275 акушерско-гинекологических, 450 детских соматических и 280 детских коек хирургического профиля. В статье анализируются основные показатели деятельности Института после внедрения обязательного медицинского страхования (2004-2009 гг.). Необходимо подчеркнуть, что объем оказываемой медицинской специализированной помощи в данном учреждении возрастает из года в год – от 27 508 пациентов в 2004 году до 32 055 в 2009 году. Средняя деятельность работы койки в году составляет 260-280 дней, средняя деятельность работы койки в году – 8,0-8,9 дней, летальность – 0,5%, удельный вес пациентов из сельской местности 62,4%. За указанные годы финансирование Института существенно возросло от 32,1 млн. лей в 2004 году до 102,1 млн. лей, предусмотренных на 2010 год, но этого явно недостаточно для учреждения ΙΙΙ уровня при оказании медицинской специализированной помощи. Далее в статье приводятся динамика и объем медицинских услуг, оказываемых пациентам, а также динамика изменений стоимости лечения одного случая. В заключении автор указывает на существующие проблемы в управлении учреждением III уровня, вносит предложения по их разрешению, а также рекомендует перспективы развития данного учреждения.
