Obiective: Lucrarea este consacrată unei probleme importante a neurochirurgiei vertebrogene contemporane - tratamentului chirurgical al herniilor de disc lombare luând pentru comparaţie rezultatele diferitor tehnici chirurgicale pentru diferite tipuri de hernii de disc lombare. Metode: a fost efectuată analiza retrospectivă a 232 pacienţi cu diagnoza de hernie de disc lombară trataţi prin discectomie microchirurgical în spitalul nostru, pe parcursul ultimilor 7 ani. Corelaţia bărbaţi/femei este de 1,1/1. Vârsta medie 47 ani. Cel mai tânăr pacient a fost de 17 ani, cel mai stimabil - 71 ani. Rezultate: Studiul următor a demonstrat în 80 % cazuri recuperare totală, în 11,3 % din pacienţi - persistenţa durerii, în 4,0 % - „failed back syndrome" şi în 4,7 % - persistenţa postoperatorie a parezei. Concluzii: Succesul tratamentului microchirurgical al herniilor discale lombare este determinat de diagnosticul perfect confirmat prin toate investigaţiile paraclinice. Tehnica chirurgicală este determinată individual. Avantajul major al discectomiilor microchirurgicale constă în iluminarea bună a câmpului operator, vizualizarea perfectă a celor mai mici structuri vasculare şi nervoase, impactul chirurgical minimal asupra aparatului ligamentar şi structurilor nervoase, reducerea riscului complicaţiilor postoperatorii şi o perioadă de reabilitare mai redusă.en_US
dc.description.abstractSummary. Objective: the aim of this study was to improve the outcome of surgery for lumbar disc herniation, comparing the results of different surgical techniques for different types of lumbar disc herniation and its surgical approach. Methodes: A retrospective analysis of 232 consecutive patients with the diagnosis of lumbar disc herniation surgically treated by microneurosurgical discectomy in our neurosurgical department during the last 7 years was performed. The correlation men/women were of 1.1/1. Average 47 years. The youngest patient was of 17 years, the oldest one - 71 years. Results: follow up study showed a recovery rate of 80 % of cases, 11.3 % of patients showed persistent pain, 4.0 % - failed back syndrome and 4.7 % - persisting postoperative paresis. Conclusions: success of microneurosurgical treatment of lumbar disc herniation is determined by the perfect diagnosis confirmed with all paraclinical investigations. Surgical approach is determined individually. The major advantage of microneurosurgical discectomies consists of good illumination of surgical field, perfect visualization of smallest vascular and nervous structures, minimal surgical impact over ligamentary apparatus and nervous structures, reduced risk of postoperative complications, short postoperative recovery period.en_US
