Particularitățile clinico-imagistice ale insuficienței cardiace asociate cu afectarea neoplazică a pericardului

Abstract

Rezumat. În articol sunt prezentate rezultatele studiului retrospectiv al fișelor de observație și al rapoartelor morfopatologice a 29 pacienți diagnosticați cu insuficiență cardiacă asociată tumorilor primare sau metastatice ale pericardului. Insuficiența cardiacă congestivă a fost cea mai frecventă manifestare a tumorilor primare și metastatice ale pericardului, fiind refractară la tratament și cu evoluție nefavorabilă. Tumorile metastatice ale pericardului au fost diagnosticate în 96,5% de cazuri. Dintre localizările primare au prevalat cancerul glandei mamare și cancerul pulmonar. Diagnosticul definitiv a necesitat o abordare multimodală care a inclus atât studii imagistice, cât și evaluarea histopatologică.

Summary. The article reflects the results of the retrospective study of the case histories and the morphopathological reports of 29 patients diagnosed with heart failure and primary or metastatic tumors of the pericardium. Congestive heart failure was the most frequent manifestation of the primary and metastatic tumors of the pericardium, which was refractory to treatment and had unfavorable outcome. Metastatic tumors of the pericardium were diagnosed in 96.5% of cases. Breast cancer and lung cancer were the most prevalent primary localizations. The definitive diagnosis required a multimodal approach that included both imaging studies and histopathological evaluation.

Резюме. Данная статья отражает результаты ретроспективного исследования 29 историй болезни и протоколов анатомо-патологического исследования пациентов, у которых был выявлена сердечная недостаточность, ассоциированная с первичными или метастатическими опухолями перикарда. Застойная сердечная недостаточность была наиболее частым проявлением первичных и метастатических опухолей перикарда, при этом отмечалась рефрактерность к проводимому лечению и неблагоприятный исход. Метастатические опухоли перикарда были диагностированы в 96,5% случаев. Среди первичных локализаций превалировали рак молочной железы и рак легких. Окончательный диагноз требовал многостороннего подхода, который включал как инструментальные, так и гистопатологические исследования.
