Aspecte fiziologice și patologice ale undelor T inversate la sportivii de performanță

Abstract

Evaluarea și monitorizarea stării de sănătate a persoanelor implicate într-o activitate fizică regulată este de o maximă importanță pentru păstrarea și promovarea unei activități sportive sigure cu o funcționalitate optimă a organismului. Scopul studiului constă în diagnosticarea precoce a manifestărilor clinice de suprasolicitare a SCV, a stărilor premorbide, dar și a unor patologii cardiace la sportivi. Unul din semnele clinice sunt dereglările proceselor de repolarizare, înregistrate pe electrocardiogramă sub formă de unde T inversate. Pentru a realiza scopul cercetării s-a efectuat un studiu transversal, descriptiv. Au fost selectate fișele medicale ale sportivilor cu devierile electrocardiografice menționate, cercetată și analizată tactica examenului medical, diagnosticul, tratamentul și monitorizarea în timp. Din totalul de 3966 de sportivi investigați, 11 sportivi au prezentat devieri ale electrocardiogramei sub formă de unde T inversate, dintre care 8 au prezentat simptome cu caracter tranzitoriu, iar la celelalte 3 persoane s-au constatat simptome cu caracter permanent. Devierile electrocardiografice sub formă de unde T inversate fac parte din dereglările cu caracter patologic care, conform literaturii de specialitate, pot fi interpretate și în contextul devierilor fiziologice ca rezultat al unui efort fizic prelungit. Eliberarea avizului medical de către medicul de medicină sportivă permite sportivilor să practice activități sportive regulate într-un mod inofensiv.

Assessment and monitoring of the state of health of those people, who are engaged in regular physical activities, has maximum importance for keeping and promoting safe sports activities, with optimal functioning of the organism. The goal of the study is early diagnosis of clinical manifestations of the cardiovascular system, premorbid states, and even of heart pathologies in athletes. A clinical sign is a disorder in repolarization processes, which are recorded in the electrocardiogram in the form of inverted T-waves. Aiming to achieve the goal of this research, there was carried on a cross-sectional, descriptive, and narrative study. There were chosen the medical record sheets of the athletes with the above deviations in the electrocardiogram. There was also research and analyzed the tactics of medical examination, diagnosing, treatment, and monitoring over the course of time. As of the total number of 3,966 examined athletes, 11 athletes presented deviations in the electrocardiogram, in the form of inverted T-waves; 8 athletes of those mentioned above had symptoms of transitory nature, and 3 people had symptoms of permanent nature. Deviations in the electrocardiogram, having the form of inverted T-waves, are deviations of pathological nature in the electrocardiogram. However, according to the specialized literature, they may be also interpreted in the context of physiological deviations, as a result of a long-time physical effort. The issuance by the sports medicine doctor of the medical opinion allows the admission of athletes to practice regular sports activities in a harmless way.

Оценка и контроль состояния здоровья тех, кто регулярно практикует физическую активность, максимально важно для поддержания и стимулирования безопасной спортивной деятельности при оптимальной работе организма. Целью исследования является диагностика клинических проявлений перегрузки сердечнососудистой системы, пред-клинических состояний и сердечных патологий у спортсменов. Одним из клинических проявлений являются нарушения процессов реполяризации, которые регистрируются на электрокардиограмме в виде перевёрнутых зубцов Т. Для реализации цели работы было проведено описательное, поперечное, нарративное исследование. Были отобраны медицинские карты спортсменов с указанными отклонениями на электрокардиограмме, также была исследована и проанализирована тактика медицинского обследования, диагностики, лечения и контроля с течение времени. Из общего числа 3966 обследованных спортсменов у 11 человек на электрокардиограмме были представлены отклонения в виде перевёрнутых зубцов Т, у 8 из указанных спортсменов симптомы носили временный характер, а у 3 были выявлены постоянные симптомы. Отклонения на электрокардиограмме в виде перевёрнутых зубцов Т являются патологическими отклонениями, однако, согласно специализированной литературе, они также могут трактоваться и в контексте физиологических отклонений как результат длительного физического напряжения. Выдача медицинского заключения спортивным врачом позволяет спортсменам безопасно заниматься регулярной спортивной деятельностью.
