Rolul fenotipului gingival în remanierele osoase peri-implantare timpurii

Abstract

Rezumat. În conformitate cu datele literaturii, conservarea osului osos periimplantar este un proces multi-factorial, iar unul din rolurile cheie revine ofertei cantitative şi calitative ale ţesuturilor moi. La necesitate, cea din urmă poate fi creată prin diverse tehnici de grefare, fie pre-implantar, la momentul inserării implanturilor, sau ulterior. Rămâne actuală determinarea influenţei fenotipului gingival asupra osului periimplantar în perioada de integrare. Scopul studiului constă în evaluarea influenţei mucoasei asupra remanierelor osoase peri-implantare în perioada de vindecare. Studiul s-a axat pe analiza resorbţiei osului periimplantar în jurul a 109 implanturi dentare demontabile inserate într-un timp chirurgical la 64 pacienţi edentaţi în sectoarele posterioare mandibulare (grupul premolar şi molar) care prezentau diferite grosimi ale mucoasei. În urma evaluării corelaţiilor a fost determinat faptul că fenotipul gingival nu are impact asupra resorbţiei osului periimplantar în perioada de vindecare. Reieşind din cele expuse, grefarea gingivală poate fi efectuată atât preimplantar sau în timpul inserării implanturilor, cât şi după o perioadă de la inserarea acestora.

Summary. According to the literature the preservation of peri-implant bone is multi-factorial process and one of the key factor is the quality and quantity of soft tissues. If necessary, it can be created by different grafting procedures before implant placement, during insertion or even after it. It is necessary to appreciate the influence of gingival phenotype upon periimplant bone loss during healing period. The aim of the study was to evaluate the soft tissue influence upon peri-implant bone remodeling during the healing period. The study was performed by analyzing the bone loss around 109 two-piece dental implants installed in one surgical step in 64 patients in posterior sides of the lower jaw (premolar and molar regions) who presented different thickness of the mucosa. After correlation analysis it was determined that the gingival phenotype have no impact on peri-implant bone loss during the healing period. Thus, gingival grafting can be performed before implant placement, during insertion or even after it.
