Tricofiţie zooantropofilă cu manifestări clinice diferite – prezentare de caz

Abstract

Generalităţi. Prezentăm cazul unui pacient, 56 de ani, locuitor rural, spitalizat în Spitalul Dermatologie şi Maladii Comunicabile, în 16 februarie 2015. La examenul clinic obiectiv, pe partea anterior-medială a antebraţului drept s-a constatat un focar de formă ovală, 6-8 cm, cu eritem bine conturat, puţin infiltrat, la periferie – vezicule cu mici cruste elevate, descuamaţie furfuracee, pustule. La examenul de laborator, microscopic s-au depistat filamente miceliene, în cultură – Trichophyton ectothrix gypseum. Sub aspect epidemiologic, este notabil faptul, că în gospodărie există iepuri cu suspiciu de micoză zooantropofilă. Tratamentul a inclus îndepărtarea mecanică a firelor de păr, alcool iodat 2%, unguent sulfo-salicilic 3%, grizeofulvină 0,125, câte 2 pastile, de 3 ori pe zi, timp de 3 săptămâni. Material și metode. Regresia focarului – peste 40 de zile. La 30 iunie 2015, la un control repetat, focarul de pe antebraţ a regresat, cu depigmentare; pe fese și părţile dorsale ale coapselor – pete roz-roșietice multiple, confluente, delimitate, cu descuamaţie furfuracee. Examinare: Trichophyton gypseum. Tratament recomandat: grizeofulvină 0,125, câte 2 pastile – de 6 ori, ung. sulfo-salicilic 3%, t-rae iodi 1%, cu rezolvarea erupţiei. Concluzii. Cazul prezintă interes pentru clinica trichofiţiei zooantropofile cu aspect de tinea inflamatorie și tinea superficială cutanată.

The author presents a case of 56 years old male from rural region, who was admitted to the Hospital of Dermatology and Communicable Diseases on the 16th of February 2015. During clinical inspections an oval, 6-8 cm in diameter, well-marked, erythemato-infiltrative plaque has been revealed; vesicles, pustules, crusts and desquamation have been seen on the periphery part of the lesions; eruptions were localized on the anterior-medial surface of the right shoulder. Microscopic examination of the scales detected filaments of mycelium and culture identified Trichophyton ecotrix gypseum. Personal history revealed that the patient was in habitual contact with rabbits which had lesions similar to trichophytosis. Treatment included mechanical extraction of the infected hair shafts, 2% iodine solution, 3% sulfur-salicylic acid, Griseofulvin 125 mg 2 pills 3 times per day, 3 weeks. Foci of infection have regressed during 40 days. On the 30th of June 2015 the patient came for follow-up control: lesion on the shoulder had disappeared, only secondary hyperpigmentation remained; but on the buttocks and inner surface of the thighs an erythematous red color, confluent, well marked macules were seen, covered with white scales. An additional therapy with Griseofulvin 125 mg 2 pills 6 times per day, 3%sulfur-salycilis acid, 1% iodine solution was administered and lead to complete regression of the lesions.
