Probleme anestezice în timpul bronhoscopiei la copii

Abstract

Rezumat În ultimii ani pneumologia și endoscopia pediatrică s-au dezvoltat semnificativ, ceea ce a condus la progrese în diagnosticul, tratamentul și prevenirea bolilor organelor respiratorii la copii. Studiul pune în discuție probleme ansteziologice întâlnite în timpul bronhoscopiei. Scopul: specificarea celor mai frecvente scenarii ale complicațiilor anestezice în timpul bronhoscopiei la copii, managementul terapeutic și analiza metodelor optime de asistență anesteziologică. Materiale și metode: studiu descriptiv retrospectiv a 164 fișe de observație a pacienților, care au necesitat asistență anesteziologică pentru procedura de bronhoscopie, spitalizați pe parcursul anului 2024 în IMSP IMC.

Summary In recent years, there has been significant development of pediatric pulmonology and endoscopy, which has led to progress in the diagnosis, treatment and prevention of respiratory diseases in children. The study discusses anesthetic problems encountered during bronchoscopy related to anatomical and physiological age-related peculiarities of the respiratory organs. Describing the most common scenarios of anesthetic complications during bronchoscopy in children, therapeutic management of complications and analysis of optimal methods of anesthetic assistance during bronchoscopic procedures performed in IMSP IMC, endoscopy department. The most common anesthetic complications were: hypoxia, bradycardia and laryngospasm. The main mechanism in the occurrence of laryngospasm and bradycardia is the vagal reflex, as a result of superficial anesthesia, and the impossibility of adequate ventilation, caused by the bronchoscope. These play a primary role in the development of hypoxia.
