CAZ CLINIC: POLIP ANTROCHOANAL CU EXTINDERE OROFARINGIANĂ LA COPIL

Abstract

Introducere. Polipii antrochoanali, formaţiuni benigne ce provin din sinusul maxilar şi se extind spre cavitatea nazală, choane şi nazofaringe, devin tot mai relevanţi în contextul creşterii incidenţei rinosinuzitelor cronice, rinitei alergice şi altor boli, necesitând o atenţie sporită la diagnostic şi tratament. Scop. Studiul particularităţilor fiziopatologice şi clinice ai polipilor antrochoanali,ai celor cu expansiune oro-faringiană, în vederea îmbunătăţirii procesului diagnostic, curativ, prezentare de caz clinic. Material şi metode. Datele clinice au fost preluate din fişa pacientului tratat la Clinica „Emilian Coţaga”, Chişinău. S-a efectuat operaţia funcţional endoscopică, evidenţiindu-se polipul antrochoanal care a fost îndepărtat împreună cu pediculul din sinusul maxilar prin utilizarea endoscoapelor 00 şi 450, urmat de revizia mucoasei şi tamponament cu Merocel. Rezultate. Pacienta în vârstă de 5 ani s-a prezentat cu obstrucţie nazală stângă, senzaţie de corp străin în gât, disfagie şi modificări ale vocii de aproximativ un an. La orofaringoscopie s-a evidenţiat o formaţiune în spatele palatului moale, deplasându-l anterior. Endoscopia nazală şi tomografia computerizată au confirmat un polip antrochoanal stâng care se extindea în orofaringe. S-a efectuat intervenţia chirurgicală funcţional endoscopică sinusală cu excizia completă a polipului, păstrând funcţiile fiziologice ale sinusului. Examenul his-topatologic a confirmat formaţiune polipoasă cu epiteliu respirator, stroma edematoasă şi infiltrat moderat. Concluzii. Acest caz ilustrează o manifestare atipică a unui polip antrochoanal cu extindere în orofaringe. Diagnosticul de predilecţie se bazează pe endoscopia nazală şi tomografia computerizată, iar tratamentul de elecţie este operaţia funcţional endoscopică şi examenul histopatologic al ţesutului prelevat.

Introduction. Antrochoanal polyps, benign formations originating from the maxillary sinus and extending in the nasal cavity and nasopharynx, are relevant in the context of the increasing incidence of chronic rhinosinusitis, allergic rhinitis and other diseases, requiring increased attention to diagnosis and treatment. Objective. Studying the pathophysiological and clinical particularities of antro-choanal polyps with oropharyngeal expansion, in order to improve the diagnostic process, clinical case presentation. Material and methods. The clinical data were taken from the patient's file treated at Emilian Cotaga Clinic, Chisinau. Functional endoscopic surgery was performed, highlighting the antrochoanal polyp which was removed together with the pedicle from the maxillary sinus using endoscopes 0o and 450, followed by mucosal revision and tamponade with Merocel. Results. Patient 5 years old boy presented with left nasal obstruction, foreign body sensation in the throat, dysphagia, and voice changes for approximately one year long. Oropharyngoscopy revealed a mass behind the soft palate, displacing it anteriorly. Nasal endoscopy and computed tomography confirmed a left antrochoanal polyp extending into the oropharynx. Functional endoscopic sinus surgery with complete excision of the polyp was performed, preserving the physiological functions of the sinus. Histo-pathological examination confirmed polypous formation with respiratory epithelium, edematous stroma and moderate infiltrate. Conclusion. This case illustrates an atypical manifestation of an antrochoanal polyp with extension in the oropharynx. The diagnosis is based on nasal endoscopy and computed tomography, and the elective treatment is functional endoscopic surgery and histopathological examination of the tissue.
