APLICABILITATEA SCALEI „MODIFIED DENTAL ANXIETY SCALE" ÎN TRATAMENTUL STOMATOLOGIC SUB ANESTEZIE GENERALĂ

Abstract

Introducere. Deşi majoritatea tratamentelor stomatologice se realizează sub anestezie locală, în anumite cazuri clinice este indicată anestezia generală. Anestezia generală se aplică frecvent la persoane cu dizabilităţi intelectuale, pacienţi adulţi şi copii necooperanţi sau cu un grad de anxietate ridicat. Scop. Evaluarea eficienţei utilizării scalei Modified Dental Anxiety Scale (MDAS) în selecţia pacienţilor anxioşi, în vederea realizării tratamentului stomatologic sub anestezie generală. Material şi metode. A fost realizată o analiză sistematică a literaturii privind utilizarea anesteziei generale în tratamentul stomatologic, consultând bazele de date ştiinţifice PubMed, ResearchGate şi Google Scholar, criteriile de includere fiind: studii clinice, revizuiri sistematice sau meta-analize, care au utilizat scala Modified Dental Anxiety Scale. Rezultate. În urma analizei a 10 articole ştiinţifice selectate conform criteriilor stabilite, s-a constatat că scala MDAS este un instrument util pentru cuantificarea anxietăţii, scoruri între 5 şi 25. În majoritatea studiilor, un scor > 19 şi lipsa cooperării pacientului, justifică utilizarea anesteziei generale. Totuşi, analiza comparativă a literaturii relevă opinii divergente privind aplicabilitatea universală a scalei. În 3 articole, MDAS nu este considerată suficientă în absenţa altor factori clinici: istoricul medical, gradul de cooperare sau complexitatea tratamentului. Astfel, MDAS trebuie utilizată ca parte a unei evaluări multidimensionale. Concluzii. Scala Modified Dental Anxiety Scale este un instrument util şi validat pentru evaluarea nivelului de anxietate, dar nu trebuie utilizată ca unic criteriu de selecţie, ci ca parte a unui proces decizional multidisciplinar în stabilirea indicaţiei pentru anestezie generală în stomatologie.

Introduction. Although most dental treatments are performed under local anesthesia, in certain clinical situations general anesthesia is indicated. General anesthesia is commonly used for patients with intellectual disabilities, uncooperative adults and children, or those with a high level of dental anxiety. Objective. Evaluation of the effectiveness of the Modified Dental Anxiety Scale (MDAS) as an effective tool for identifying highly anxious patients for dental treatment under general anesthesia. Material and methods. A systematic review of the literature on the use of general anesthesia in dental treatment was conducted by consulting scientific databases such as PubMed, ResearchGate, and Google Scholar. Inclusion criteria consisted of clinical studies, systematic reviews, or meta-analyses that used the Modified Dental Anxiety Scale (MDAS). Results. Following the analysis of 10 scientific articles, it was found that the MDAS is a useful tool for quantifying dental anxiety, with scores 5 to 25. In most studies, a score > 19 and lack of cooperation were considered clinical justification for the use of general anesthesia. The comparative analysis of the literature reveals divergent opinions regarding the applicability of the scale. In 3 articles, the MDAS was not deemed sufficient to justify general anesthesia in the absence of other clinical factors such as medical history, level of cooperation, or procedural complexity. The MDAS should be used as part of a multidimensional evaluation. Conclusion. The MDAS is a useful and validated instrument for assessing the level of dental anxiety, but it should not be used as the sole selection criterion. Instead, it should be integrated into a multidisciplinary decision-making process when determining the indication for general anesthesia in dentistry.
