Opţiuni şi limite în corectarea deficienţei transversale maxilare

Abstract

Introducere. Deficienţa transversală maxilară este o anomalie dento-maxilară ce implică dezvoltarea insuficientă în plan transversal a maxilarului superior. Varietăţile de tratamentul ortodontic presupun remodelarea bazei osoase şi a arcadelor dentare, în funcţie de vârsta şi gradul de osificare a suturii palatine. Scop. Analiza metodelor de tratament ortodontic utilizate în corectarea deficienţei transversale maxilare, evidenţiind aplicabilitatea, indicaţiile şi limitele anatomo-fiziologice. Material şi metode. Revizuire narativă a literaturii de specialitate şi selectarea varietăţilor de tratament ortodontic utilizate în corectarea deficienţei transversale maxilare: plăcuţe cu şurub, aparatele mobi-lizabile, expansiunea rapidă (Hyrax, Haas), MARPE, SARPE şi metode clasice (extracţii selective), evidenţiind posibilităţile şi limitele acestora. Rezultate. Tratamentul deficienţei transversale variază cu vârsta şi depinde de gradul de maturare a suturii palatine. La copii (3-6 ani), plăcile cu şurub şi aparatele funcţionale stimulează transversal dezvoltarea maxilarului, într-o manieră interceptivă. În dentiţia mixtă (6-12 ani), expansiunea rapidă cu Hyrax, Haas induce separarea suturii şi dezvoltare transversală cu efecte schele-tale stabile. În adolescenţă şi adulţi tineri, MARPE extinde aplicabilitatea tratamentului ortodontic, fiind eficientă în suturi parţial osificate. La adulţi, SARPE permite obţinerea rezultatelor durabile, chiar şi în cazurile cu osificare completă a suturii mediane. Concluzii. Varietatea aparatelor ortodontice permit selectarea adaptată vârstei şi gradului de osificare a suturii palatine. Aparatele mobile, plăcile cu şurub sunt eficiente la copii, RME stimulează expansiunea scheletală în dentiţia mixtă, iar MARPE, SARPE oferă rezultate stabile în adolescenţă şi la adulţi.

Introduction. Transverse maxillary deficiency is a den-to-maxillary anomaly involving insufficient transverse development of the upper jaw. The variety of orthodontic treatment options aims to remodel the bony base and dental arches, depending on the patient's age and the degree of ossification of the palatal suture. Objective. To analyze the orthodontic treatment methods used to correct transverse maxillary deficiency, highlighting their applicability, indications, and anatomical-physiological limitations. Material and methods. A narrative review of the specialized literature and selection of orthodontic treatment modalities used in the correction of transverse maxillary deficiency: screw-activated plates, removable appliances, rapid maxillary expansion (Hyrax, Haas), MARPE, SARPE, and classical methods (selective extractions), emphasizing their potential and limitations. Results. Treatment of transverse deficiency varies with age and depends on the maturation degree of the palatal suture. In children (ages 3-6), screw-activated plates and functional appliances stimulate transverse maxillary development in an interceptive manner. In mixed dentition (ages 6-12), rapid maxillary expansion using Hyrax, Haas appliances induces suture separation and transverse skeletal development with stable effects. In adolescents and young adults, MARPE extends treatment options, being effective in partially ossified sutures. In adults, SARPE achieves stable results, even in cases with complete ossification of the midpalatal suture. Conclusion. The variety of orthodontic appliances allows selection adapted to age and palatal suture ossification. Removable appliances, screw-activated plates are effective in children, RME stimulates skeletal expansion in mixed dentition, and MARPE, SARPE offer stable outcomes in adolescents and adults.
