Abstract

Actualitatea cercetării: În ultimii ani, în literatura de specialitate se remarcă un interes deosebit pentru studierea metodelor de tratament ale chisturilor maxilare voluminoase. Acest fapt se explică prin existenţa unor controverse privind alegerea metodei de tratament, conduita faţă de dinţii limitrofi chistului și faţă de formaţiunile anatomice învecinate. Scopul lucrării: Creșterea eficacităţii asistenţei medicale acordate pacienţilor cu chisturi voluminoase prin studierea, compararea și optimizarea metodelor minim invazive de tratament. Obiectivele cercetării: Analiza retrospectivă a morbidității pacienților cu formațiuni chistice ale maxilarelor; evaluarea clinică comparativă a recuperării pacienților în perioada postoperatorie; determinarea variațiilor sensibilității pulpare a dinților adiacenți leziunii chistice, pre- și postoperator, în funcție de metoda terapeutică și localizarea anatomică a formațiunii; studiul comparativ al densității osoase și tisulare în funcție de tipul de tratament și localizarea anatomică a formațiunii chistice; compararea pre- și postoperatorie a eficienței decompresiei și marsupializării în reducerea parametrilor morfologici ai chisturilor maxilare voluminoase. Noutatea şi originalitatea ştiinţifică: Au fost studiate, comparate și optimizate metodele minim invazive de tratament ale pacienților cu chisturi maxilare voluminoase. Rezultatele majore noi obținute: Ambele metode de tratament pot fi recomandate ca opțiuni terapeutice sigure și eficiente în managementul leziunilor chistice maxilare voluminoase. Semnificaţia teoretică: Aplicarea metodelor minim invazive în tratamentul pacienților cu chisturi maxilare voluminoase va contribui la reducerea semnificativă a incidenței accidentelor intraoperatorii și a complicațiilor postoperatorii. Valoarea aplicativă a lucrării: Rezultatele studiului vor sta la baza elaborării recomandărilor destinate medicilor practicieni în vederea selectării metodei optime de tratament. Implementarea rezultatelor ştiinţifice: Rezultatele studiului au fost implementate în procesul didactic pentru instruirea studenților, rezidenților și a medicilor cursanți în cadrul Catedrei de chirurgie oro-maxilo-facială „Arsenie Guțan”. Rezultatele cercetărilor sunt utilizate de către medicii Centrului Republican de Chirurgie Oro-Maxilo-Facială din Republica Moldova. Structura tezei: introducere, patru capitole, concluzii generale şi recomandări, bibliografie din 219 lucrări, 5 anexe, 92 de pagini de text de bază, 70 de figuri, 3 tabele, 1 formulă. Rezultatele obţinute sunt publicate în 27 de lucrări ştiinţifice.

Relevance of the research: In recent years, the specialized literature has shown a marked interest in studying the treatment methods for large maxillary cysts. This is explained by the existence of controversies regarding the choice of treatment method, the management of the teeth adjacent to the cyst, and the approach to the neighboring anatomical structures. Objective of the study: To enhance the effectiveness of medical care provided to patients with large maxillary cysts by studying, comparing, and optimizing minimally invasive treatment methods. Research objectives: Retrospective analysis of morbidity in patients with cystic formations of the maxillae; comparative clinical evaluation of patient recovery during the postoperative period; determination of variations in pulpal sensitivity of teeth adjacent to the cystic lesion, pre- and postoperatively; comparative study of bone and tissue density; comparison of pre- and postoperative efficacy of decompression and marsupialization in reducing the morphological parameters of large maxillary cysts. Scientific novelty and originality: The minimally invasive treatment methods for patients with large maxillary cysts have been studied, compared, and optimized. Major new results obtained: Both treatment methods can be recommended as safe and effective therapeutic options for the management of large maxillary cystic lesions. Theoretical significance: The application of minimally invasive methods in the treatment of patients with large maxillary cysts will contribute to a significant reduction in the incidence of intraoperative accidents and postoperative complications. Practical significance of the study: The results of the study will serve as a basis for developing recommendations for practicing physicians to guide the selection of the optimal treatment method. Implementation of scientific results: The results have been implemented in the educational process for the training of students, residents, and physicians in the Department of Oral and Maxillofacial Surgery. The research findings are utilized by the physicians of the Republican Center of Oral and Maxillofacial Surgery in the Republic of Moldova. Structure of the thesis: Introduction, four chapters, general conclusions and recommendations, bibliography of 219 works, 5 appendices, 92 pages of main text, 70 figures, 3 tables, 1 formula.
