Human bocavirus infection (HBoV) is a relevant cause of respiratory tract infections in the pediatric population. It may be associated with prolonged clinical courses and diagnostic challenges, particularly in children with complex underlying comorbidities. We present the case of a child with multiple comorbidities and significant medical history, in whom the diagnosis of human bocavirus infection was established in the setting of prolonged fever and a persistent inflammatory syndrome. According to the medical history, the child had been considered ill for approximately three months, presenting with marked pallor, apathy, decreased appetite, and low-grade fever. One week prior to admission, a productive cough developed, followed by fever reaching 38.6°C. Initial investigations performed at a regional healthcare facility revealed severe anemia (hemoglobin 30g/L), prompting transfer to a tertiary care center. The etiological diagnosis was established using a multiplex respiratory panel, which detected human bocavirus infection. This case highlights the importance of incorporating modern molecular diagnostic methods into the evaluation of children with comorbidities (congenital ichthyosis) and atypical clinical evolution, facilitating accurate identification of the infectious etiology and optimization of therapeutic management.en_US
