Background: Glaucoma is the only eye disease classified as a chronic disease, amongst the legislated 25 chronic diseases. It is considered by the medico-social community as one of the most serious problems in the world resulting in blindness. It is estimated that about 70 million people suffer from glaucoma in the world. The chronic pain in this orbital region is a complex phenomenon. The management of this type of pain is complicated including a correct diagnosis and searching the primary causes. Enucleation is the final treatment for terminal painful glaucoma. Material and methods: Our study included 208 patients that were divided in two groups. The study group consisted of 104 patients (104 eyes), it was divided in two groups of 60(study I) patients and 44(study II) patients treated with the combined antihypertensive, NSAID, and ascorbic acid. Control group (104 patients, 104 eyes) was also divided into groups with the same number of patients who were given anti-glaucoma treatment. Results: The results demonstrated the efficiency of the combined treatment in pain relief and intraocular pressure in painful glaucoma. Administration a systemic NSAID and a local eye drops with ascorbic acid can modify the tears pH in painful glaucoma. Conclusions: Our research reported that the patients with the painful syndrome in painful glaucoma usually associated with marked ophthalmohypertension, alkaline level in tears and redness. Our study allowed us to elaborate a complex treatment algorithm for combating the painful syndrome in painful glaucoma.en_US
