Haemostasis is a waterfall of physiological phenomena, overlapped in time, that appear during the haemorrhage stopping by vasomotor, thrombocyte and plasmatic mechanisms. The haemostasis system includes: the primary and  secondary haemostasis (coagulation). The primary haemostasis represents a succession of reactions that induce a temporary stopping of bleeding allowing the realization of the conditions needed to forming the final clot. Primary haemostasis includes parietal and thrombosis time.&#13;
Parietal time- is represented by the phenomena through which the blood vessel lumen decreases thus reducing or temporary stopping the bleeding of the injured vessel. Thrombosis time - represented by the thrombocyte “the morphological dwarf, the physiological giant” that under normal conditions does not adhere to unimpaired endothelium with the white thrombocyte formation in peripheral vascular bed. Primary haemostasis research, detection of the precocious disturbances and interference in due time, forewarn the decomposition of vaso-thrombosis mechanism and the protection of plasmatic mechanism.&#13;

Hemostaza este o cascadă de fenomene fiziologice, suprapuse în timp, care îşi dau concursul la oprirea hemoragiei prin mecanisme vasomotorii, trombocitare şi plasmatice. Sistemul de hemostază include: hemostaza primară (timpul vaso-plachetar) şi hemostaza secundară (coagularea).Hemostaza primară (timpul vaso-plachetar) reprezintă o succesiune de reacţii care induc oprirea provizorie a sîngerării, permiţînd realizarea condiţiilor necesare formarii cheagului definitiv. Hemostaza primară include timpul parietal şi trombociatr. Timpul parietal este reprezentat de fenomenele prin care lumenul vasului se micşorează, reducând sau stopând momentan hemoragia din vasul lezat. Timpul trombocitar- reprezentat de trombocit ,,piticul morfologic, gigantul fiziologic”, care în condiţii normale nu aderă la endoteliul intact cu  formarea trombului alb trombocitar in patul vascular periferic. Cercetarea hemostazei primare, detectarea perturbărilor precoce şi intervenirea la timp, previn decompensarea mecanismului vaso-trombocitar şi protecţia mecanismului plasmatic.

<title>Predicţia preoperatorie a riscului cardiovascular global la pacienţi hipertensivi prin metoda „ecocardiografia Doppler”</title>

It was made a prospective study on 95 patients with various surgical pathologies, whose overall condition was aggravated with concomitant-HTA primary cardiovascular disease. In order to predict the pre-surgical cardiovascular risk there were assessed some important clues with the help of ecocardiography. Survey results have served to initiate an aggressive antihypertensive treatment to 28 patients with essential HTA of II-III degree, or careful review of the previously used scheme.&#13;

S-a efectuat un studiu prospectiv pe 95 de bolnavi cu diferite patologii chirurgicale, starea generală a cărora a fost agravată cu afecţiunea cardiovasculară concomitentă-HTA primară. Pentru predicţia riscului cardiovascular global preoperator au fost evaluaţi cîtiva indici importanţi cu ajutorul ecocardiografiei. Rezultate studiului au servit la iniţierea unui tratament antihipertensiv agresiv la 28 pacienţi cu HTA esenţială de gr. II-III, sau revizuirea atentă a schemei anterior folosite din cauza hipertrofiei marcate a ventricului stîng-factor predictiv pentru morbiditatea şi mortalitatea cardiovasculară.

CID syndrome treatment can not be conceived without pathology therapy that leds to its appearance. Treatment measures of basic affections, applied in time and necessary volume sometimes can lead to the correction or even solution of CID. These measures  selection and methods that do not affects CID syndrome by negative action on the systems that depends on the selection evolution depends (microcirrculation, haemostatic and fibrinolitic system) requires deep knowledge in the clinical pharmacology, transfuziology and intensive therapy.&#13;

Tratamentului sindromului CID nu poate fi conceput fără terapia patologiei care a condus la apariţia lui. Măsurile de tratament ale afecţiunii de bază, aplicate la timp şi în volumul necesar, pot duce uneori la corecţia sau chiar la rezolvarea CID. Selectarea acelor măsuri şi metode, care nu agravează sindromul CID prin acţiune negativă asupra sistemelor de care depinde evoluţia lui (microcirculaţia, sistemul hemostatic, fibrinolitic, etc) necesită o cunoaştere profundă a farmacologiei clinice, transfuziologiei, terapiei intensive.

<title>Profilaxia pneumoniei asociate ventilaţiei mecanice</title>

<summary type="text">Profilaxia pneumoniei asociate ventilaţiei mecanice

Nonfarmacological measures for preventing VAP are: orotracheal intubation, monitoring endotracheal tube cuff pressure, subglottic secretions drainage, avoiding delays in extubation, re-intubation, noninvasive mechanical ventilation, early tracheostomy, closed tracheal suctioning system, sterilisation or disinfection of reusable respiratory devices, barrier measures, semirecumbent position. Pharmacological measures for preventing VAP are: oral cleaning and decontamination, stress ulcer prophylaxis, avoidance of sedation and paralytic agents.&#13;

Măsuri nonfarmacologice de profilaxie  a VAP sunt: intubarea orotraheală, menţinerea presiunii în balonasul sondei traheale 20-30 cm apa, aspiraţia continuă  a regiunii subglotice, detubarea precoce, evitarea reintubării, efectuare ventilaţiei mecanice noninvazive, traheostomiei precoce, sterilizarea şi dezinfectarea dispozitivelor reutilizabile, utilizarea măsurilor de barieră, poziţionarea în semidecubit. Măsuri farmacologice de profilaxie  a VAP sunt: modularea colonizării orofaringiene şi decontaminarea, profilaxia hemoragiei digestive de stress,  evitarea sedării şi a relaxantelor neuromusculare.
