<title>Romanian translation and validation of vascular quality of life questionnaire “VascuQOL-6” in patients with lower extremity arterial disease</title>

Background: Patient  reported  outcomes  are  valuable  components  in  the  assessment of  results  of treatment  for peripheral  arterial disease  (PAD). The  aim of  the study  was totranslate  the  six  item  Vascular  Quality  of  Life  Questionnaire  (VascuQoL-6)  survey  intoRomanian,  and  to  validate  the  psychometric  performance  of  the  questionnaire  in  a representative cohort of patients with lower extremity arterial disease. Material  and  Methods: Translation  of  the  VascuQoL-6  questionnaire  was  performed following  accepted  methodology. The  overall  validation  cohort  included 100 patients  withPAD (86% with chronic limb-threatening ischemia) undergoing lower limb revascularization.In  20  patients  with  stable  PAD  two  questionnaires  were  offered  preoperatively  with  a median interval of 15 days. Another  22  patients  were  re-tested  after revascularization at amedian interval of 30 days. Results: The median time required for completion of the VascuQoL-6 survey was 2 (IQR 2-3) minutes. The translated version demonstrated high internal consistency (Cronbach’s alpha– 0.81) and there was no difference in the preoperative median VascuQoL-6 scoring duringthe test re-test assessment. Area under the ROC curve for ability to discriminate intermittentclaudication  from  chronic limb-threatening  ischemia was  0.897.  The median  VascuQoL-6score increased from 10 (IQR 8-12) points preoperatively to 18.5 (IQR 14.7-20) points post-operatively (p &lt; 0.0001) with a standardized response mean of 2.94. Conclusion: The Romanian version of the VascuQoL-6 survey demonstrated good reliability,validity and  responsiveness  and can thus be  recommended  for use in patients  with  lowerlimb PAD.

Background. It is suggested that free-floating form of deep&#13;
venous thrombosis, especially that with proximal location, is&#13;
associated with a higher rate of pulmonary embolism. The&#13;
aim of current study was to estimate the risk of symptomatic&#13;
embolism occurrence in pts with free-floating thrombus and&#13;
to perform comparative analysis of early results of conservative versus surgical treatment in these cases. Methods. In a&#13;
prospective investigation were included 69 pts (mean age:&#13;
56,82 years) with free-floating thrombi, defined by duplex&#13;
ultrasound as presence of unattached to vein wall thrombotic apex with clearly visualized oscillatory movement.&#13;
Fifty-two pts were treated surgically by means of venous plication (n=38; 73,07%) or ligation (n=14; 26,92%) asociated&#13;
in some cases with partial thrombectomy (n=11), remaining&#13;
17 pts receiving anticoagulation treatment (adjusted doses of&#13;
low molecular weight heparins with subsequent conversion&#13;
to vitamin K antagonists). Results. Frecvency of simptomatic&#13;
pulmonary embolism at admission or during hospitalization was 11,59%, while rate of diagnosed episodes of embolization&#13;
rise to 18,84. The incidence of symptomatic pulmonary embolism in conservative group was 23,52% vs. 0% - recorded&#13;
postoperatively (p&lt;0,05; t-test), but rate of embolism-related&#13;
death was 11,76% vs. 0%, respectively (p-NS; t-test). Conclusions. It is necessary to clarified and unified diagnostic criteria&#13;
of free-floating thrombus. Until then clinical significance of&#13;
free-floating thrombi cannot be objectively evaluated. Efficacy&#13;
of both surgical and conservative treatment for prevention of&#13;
massive pulmonary embolism in pts with free-floating thrombosis needs to be established on a larger clinical cases group.

The primary superficial venous aneurysms of the foot are very rare. A 34-year-old female patient developed a dorsal foot mass during the second trimester of pregnancy with no history of previous trauma, puncture, or infection. One year later, she was referred to the surgical department for excision of “foot hygroma.” Based on the clinical findings, the venous aneurysm was suspected and duplex ultrasound confirmed the diagnosis of the aneurysm of the medial marginal vein of the foot. Excision of aneurysm with bipolar ligation of marginal vein was performed under local anesthesia. The postoperative evolution was uneventful. The authors hope that the presented case report will increase the awareness of general practitioners, dermatologists, and surgeons regarding the superficial venous aneurysms of lower limbs.

Boala varicoasă a membrelor inferioare are o creştere a incidenţii la nivel mondial în ultimii ani. În pofida faptului că au apărut noi metode de tratament sau au fost modernizate cele vechi, rata recidivei postoperatorii a maladiei varicoase rămâne înaltă şi nu are tendinţă spre scădere. Dacă în diagnosticul clinic şi imagistic au fost stabiliţi piloni de consens asupra conduitei operatorii a bolii varicoase, atunci datele actuale din literatură suferă nişte lacune a uniformităţii, atunci când se propune de a defini recidiva bolii varicoase, timpul apariţiei şi metodele folosite pre- şi post operatorii de a scădea rata acesteia. &#13;

Varicose veins of the lower limbs have an increased incidence worldwide in recent years. Despite the fact that there were new methods of treatment or were upgraded old ones, postoperative recurrence rate of varicose veins remains high and has not a tendency to decrease. If in the clinical and imaging diagnosis have been established points of consensus on the operative management of varicose veins, then the current data in the literature suffer from some uniformity gaps, when it is proposed to define recurrence of varicose veins, time of appearance and the methods used pre- and postoperative to reduce its rate. &#13;

Частота варикозного расширения вен нижних конечностей в последнее время растет во всем мире. Несмотря на появление новых методов лечения и улучшение старых, частота рецидива варикозной болезни остается высокой и не имеет тенденции к снижению. Если в клиническом и диагностическом плане был достигнут консенсус по методике операции при варикозном расширении вен, то понятие рецидива варикозной болезни, сроки появления и методы снижения его частоты остаются спорными по данным современной медицинской литературы.
