Cancerul de sân este lider printre cauzele deceselor la femei. Evoluția metastatică a acestuia se caracterizează prin
instabilitatea profi lului molecular. Scopul lucrării: evaluarea expresiei citokeratinei bazale CK5 în procesul de metastazare a carcinomului mamar ductal invaziv de tip NOS (neclasifi cat altfel). Material și metode: 135 de paciente în stadii
clinice avansate, fără chimio/radioterapie antecedent eseului chirurgical. Specimenele au fost evaluate histologic (tipul,
gradul), imunohistochimic (markerul CK5/XM26), datele au fost supuse analizei statistice. Rezultate: carcinomul mamar
de tip NOS în majoritatea cazurilor este CK5 negativ (119 cazuri/88,1%). Expresia CK5 în sediul limfonodal a fost una
diferită în 8 cazuri/8,8% de cea din tumora primară. Concluzii: Odată cu înaintarea în vârstă scade gradul de diferențiere
al carcinomului ductal invaziv de tip NOS. Această tumoare în majoritatea cazurilor nu expresează citokeratina bazală
CK5. Markerul dat este instabil pe parcursul metastazării, evoluând atât în direcția achiziționării, cât și pierderii expresiei,
ultimul tip de transfer constituind majoritatea.
Nowadays, mammary carcinoma is the leading cause of deaths among women. Its metastatic evolution is characterized by the instability of molecular profi le. The Aim: assessment of basal cytokeratin CK5 expression in the metastatic
process of mammary ductal invasive carcinoma of NOS (Not Otherwise Specifi ed) type. Material and methods: 135
patients with clinically advanced mammary carcinoma, with no chemo/radiotherapy before the surgical intervention. Specimens were evaluated histologically (type, grade) and immunohistochemically (marker CK5/XM26), obtained data have
been statistically analyzed. Results: mammary carcinoma of the NOS type in the most of the cases is CK5 negative (119
cases/88,1%). Expression of CK5 in the lymph nodes was different in 8 cases (8,8%) in contrast to the primary tumor.
Conclusions: as older, the grade of differentiation in mammary carcinoma of the NOS type tends to decrease. In most of
the cases, this type of tumor does not express CK5. This marker is not stable during the metastasizing and might be either
acquired or lost, the last type of transfer being more common.
На сегодняшний день, рак молочной железы (РМЖ) является главной причиной смертности среди женского
населения. Эволюция метастазов характеризуется нестабильностью молекулярного профиля опухолевых клеток.
Цель исследования: определение экспрессии базального цитокератина CK5 в процессе метастазирования инфильтративного протокового РМЖ NOS (Not Otherwise Classifi ed) типа. Материал и методы исследования: 135
случаев в поздней клинической стадии, без химио/радиотерапии предшествующей хирургическому вмешательству. Образцы были исследованы гистологически и иммуногистохимически, используя маркер CK5/XM26. Полученные данные были статистически обработаны. Результаты: инфильтративный протоковый РМЖ NOS типа
в большинстве случаев (n=119/88,1%) CK5 отрицателен. Экспрессия CK5 в лимфатических узлах отличалась от
первичной опухоли в 8 случаях (8,8%). Выводы: с возрастом, степень дифференциации инфильтративного протокового РМЖ NOS типа имеет тенденцию к уменьшению. В большинстве случаев, этот вид РМЖ CK5 отрицателен. Данный маркер нестабилен и в процессе метастазирования, его экспрессия может быть приобретена либо
утеряна, последний вид эволюции, будучи более характерным.
