Statutul metastatic al nodulilor limfatici este unul din factorii primordiali de diagnostic și pronostic în carcinomul
mamar invaziv. Procesul de metastazare se realizează atât prin intermediul vaselor limfatice preexistente, cât și celor
neoformate la nivelul tumorii primare, spre nodulii limfatici regionali (axilari în cele mai dese cazuri). Astfel, determinarea invaziei limfatice vasculare (ILV) intra- și peri-tumoral, pe specimenele histologice devine extrem de utilă. Scopul
studiului. Analiza comparativă a distribuției vaselor limfatice la nivelul plajelor peritumorală și celei intratumorale cu
determinarea gradului ILV; și compararea datelor obținute prin metoda histologică (hematoxilina și eozină), cu cele determinate imunohistochimic (aplicarea markerului limfatic D2-40). Materiale și metode. Material morfologic prelevat de
la 74 paciente, diagnosticate cu carcinom mamar ductal invaziv de tip NOS, colectate la IMSP Institutul Oncologic din
Republica Moldova în perioada anilor 2012-2014. Determinarea gradului histologic de diferențiere celulară a fost realizat
prin metoda histologică. Prin metoda imunohistochimică a fost studiată expresia D2-40 în ariile intra și peritumorale.
Rezultate. Analiza statistică a numărului de vase din aria periferică a tumorii (D2-40p.tum) cu cele intra-tumorale (D2-40
i.tum) a scos în vizor o corelație statistic semnifi cativă (PCC-0.52; p<0.001). Altă corelație semnifi cativă a fost evidentă
dintre numărul vaselor D2-40 pozitive invadate (D2-40-ILV) cu densitatea vaselor limfatice intra-tumorale (D2-40i.tum)
(PCC-0.51; p<0.001) și peri-tumorale (D2-40 p.tum) (PCC-0.63; p<0.001). Aceeași procedură de analiză statistică a
ILV evaluat prin metoda histologică cu ILV determinat prin aplicarea D2-40 nu au prezentat corelație statistică veridică.
Concluzii. D2-40 este un marker limfangiogenic selectiv, foarte util în stabilirea calitativă și cantitativă a gradului de ILV.
Interpretarea ILV prin metoda clasică poartă caracter ambiguu și necesită minuțiozitate în diagnostic. Densitatea limfatică
intratumorală este cu atât mai intensă cu cât crește densitatea peri-tumorală. Gradul de invazie limfatică vasculară este cu
atât mai acerb cu cât densitatea vaselor limfatice este mai mare.
The status of metastatic lymph nodes is one of the primary diagnostic and prognostic factors in the invasive breast
carcinoma. The process of metastasis is achieved through the pre-existing lymphatic vessels and by the new one formed
at the level of the primary tumor, to the regional nodes (axillary in the most of cases). So the determination of the intra-tumoral and peri-tumoral lymphatic vascular invasion (ILV) on the histological specimens is extremely useful. Objective
of the study. The comparative analysis of the lymphatic vessels distribution in peri-tumoral and intra-tumoral areas with
the determination of the ILV degree, as well the comparison of the data obtained by the histological method (hematoxylin
and eosin) with those determined by the immunohistochemical method (application of the lymphatic marker D2-40).
Materials and methods. The morphological material, collected from IMSP Oncological Institute of the Republic of Moldova during the 2012-2014 years was taken from 74 patients. All cases were morphologically described as invasive ductal
breast carcinoma of NOS type. The determination of histological differentiation of the tumors was performed with the
histological method. The D2-40 in the intra-and peritumoral areas was studied by immunohistochemical method. Results.
Statistical data of peri-tumoral and intra-tumoral concentration of D2-40 revealed a signifi cant correlation. (PCC-0.52;
p<0.001). Another signifi cant correlation was obvious in case of invaded vessels (which are D2-40 positive) (D2-40-
ILV), intra-tumoral density of lymphatic vesseles (PCC-0.51; p<0.001) and peri-tumoral density of lymphatic vessels
(PCC-0.63; p<0.001). The same analysis of ILV by histological and immunochemaical methods didn’t reveal a statistic
correlation. Conclusions. The D2-40 is a selective lymphangiogenic marker, which is very useful in determining the qualitative and quantitative degree of the ILV. The interpretation of the classical method for ILV is ambiguous and requires a
thorough diagnosis. The intratumoral lymphatic density is higher simultaneously with the increasing of the peri-tumoral
one. The lymphatic invasion becomes much more intense when the vascular density is higher.
Наличие метастаз в лимфатических узлах является наиболее важным фактором диагноза и прогноза инвазивного рака молочной железы. Метастазирование происходит как за счет существующих, так и за счет новообразованных лимфатических сосудов. Таким образом, определение инвазии лимфатических сосудов (ILV), интра- и
перитуморальных, на гистологических срезах становится чрезвычайно полезным. Цель исследования. Сравнительный анализ распределения интра- и перитуморальных лимфатических сосудов с определением степени ILV;
и cравнение данных, полученных при помощи гистологического метода (окраска гематоксилином и эозином) с
данными имуногистохимического метода (маркер D2-40). Материалы и методы. Первичные опухоли, извлеченные у 74 пациенток с установленным морфопатологическим диагнозом: протоковый инвазивный рак молочной
железы NOS, собранные в онкологическом институте Молдовы на протяжении 2012–2014 годов; определение степени клеточной дифференцировки производилось при помощи гистологического метода. Иммуногистохимическим методом была изучена экспрессия D2-40 в интра- и перитуморальных участках опухоли. Результаты. Статистический анализ перитуморальных лимфатических сосудов (D2-40 p.tum) с внутритуморальными (D2-40 i.tum)
выявило статистическую значимую корреляцию (PCC-0.52; p<0.001). Другая значимая корреляция проявляется
при D2-40 положительных инвазированых сосудов (D2-40-ILV) между плотностью интратуморальных лимфатических сосудов (D2-40i.tum) (PCC-0.51; p<0.001) и перитуморальных (D2-40p.tum) (PCC-0.63; p<0.001). Анализ
ILV (по интерпретации гистологическим методом) и ILV при применении D2-40 не выявило статистическую
корреляцию. Выводы. D2-40 является селективным маркером лимфангиогенеза, очень полезным при качественном и количественном определении ILV. Интерпретация ILV гистологическим методом требует скрупулезности и
является двусмысленным. Чем выше плотность перитуморальных лимфатических сосудов, тем выше плотность
интратуморальных лимфатических сосудов. Степень ILV зависит от плотности лимфатических сосудов.
