Background: Surgical management of patients with pancreatic pseudocyst remains a diFficult problem,
the causes being obscure pathogenesis, unpredictable evolution, high lethality in cases of development
of serious complications, controversies in diagnosis criteria and therapeutic options. Objective of the
study. Evaluation of the results of the strategy and surgical management applied in the treatment of
patients with pancreatic pseudocyst (PP) Material and Methods. The study presents the results of the
surgical treatment applied to 347 patients with pancreatic pseudocyst, carried out during the years 1992-
2020 in the Surgery Clinic No.2, IMSP SCR „Timofei Moșneaga” Results.surgeries performed:
cystopancreatojejunostomy (CPJS) on the loop by Roux215(61,96%) cases, external drainage of PP10
(2,88%) cases, choledocojejunostomy on a speculate loop by Roux 37(10,66%) cases, caudal
pancreatectomy with pancreaticojejunostomy, splenectomy 5 (1,44%) cases, enucleation of pancreatic
cyst – 1 (0,29%) case, cephalicduodenopancreatectomy – 2 (0,58%) cases. The patients evolution was
favourable in 277(80,0%) cases. The rate of early complications - 28 (8,06%) cases. Late complications15 (4,32%) cases reason for 16(4,61%) of them required a new classical surgery and 10 (2,88%) a
minimally invasive. Lethality was 1 (0,29%)cases postoperatively. Conclusion. The severity of
clinically assessed pancreatic lesions, laboratory and imaging results versus associated pathologies
argues an individual, multidisciplinary approach to the patient with pancreatic pseudocyst.
Introducere. Managementul chirurgical al pacienţilor cu pseudochist pancreatic rămâne o problemă
dificilă, cauzele fiind patogeneza obscură, evoluţia imprevizibilă, letalitatea înaltă în cazurile dezvoltării
complicaţiilor grave, controversele în criteriile diagnostice şi opţiunile terapeutice. Scopul lucrării.
Evaluarea rezultatelor strategiei și a managementului chirurgical aplicat în tratamentul pacienților cu
pseudochist de pancreas (PP) Material și Metode. Studiul prezintă rezultatele tratamentului chirurgical
aplicat la 347 pacienţi cu pseudochist pancreatic, desfăşurat în perioada anilor 1992-2020, în Clinica
Chirurgie nr. 2, IMSP SCR „Timofei Moșneaga” Rezultate. Operaţiile efectuate: chistpancreaticojejunostomie (CPJS) pe ansă Roux-215(61,96%) cazuri; drenare externă a PP-77(22,19%) cazuri;
drenare ecoghidată a PP-10(2,88%) cazuri; CPJS, coledoco-jejunostomie pe ansă bispiculată a lá Roux–
37(10,66%) cazuri; pancreatectomie caudală cu pancreaticojejunostomie, splenectomie-5(1,44%)
cazuri, enucleerea chistului pancreatic 1(0,29%) caz; DPC-2(0,58%) cazuri. Evoluţia pacienţilor a fost
favorabilă în 277(80,0%) cazuri. Rata complicaţiilor precoce - 28(8,06%) cazuri, tardive-15(4,32%)
cazuri. Au necesitat reintervenţie chirurgicală clasică-16(4,61%) pacienţi şi miniinvazivă-10(2,88%)
pacienţi. S-a semnalat un 1(0,29%) caz de deces postoperator. Concluzii. Gradul severității leziunilor
pancreatice apreciate clinic, rezultatele de laborator și imagistice versus a patologiilor asociate,
argumentează o abordare individuală, multidisciplinară a pacientului cu pseudochist pancreatic.
