În acest articol se dezvăluie cele mai importante mecanisme și biomarkeri ai remodelării post-infarct a miocardului. Sunt evidențiate evenimentele patofiziologice care au avut loc în faza acută a infarctului de miocard, precum răspunsul inflamator și activarea stresului oxidativ și a metaloproteinazelor matricei extracelulare în zona de vindecare a necrozei și hipertrofie a miocardului viabil – elementele-cheie ale remodelării miocardice. Un punct de vedere special constă în aprecierea mecanismelor și instrumentelor fibrozei și hipertrofiei miocardice care au loc în timpul perioadei de remode-lare post-infarct și durează circa 4-6 luni. De asemenea, se accentuează ceea ce stă la bazele remodelării patologice și se manifestă prin dilatarea ventriculară și înrăutățirea insuficienței cardiace. În ceea ce privește panoul multimarker în diagnosticul și prognoza cardiovasculară, s-a conchis că cei mai semnificativi predictori ai remodelării post-infarctice a miocardului nu au fost dezvăluiți.
In this article the most important mechanisms of post-infarction remodelling of myocardium as well as the inherent biomarkers are disclosed. There are underlined the pathophysiologic events occurred in the acute phase of infarction such as inflammatory response, and activation of oxidative stress and extracellular matrix metalloproteinases triggering the necrotic zone healing and hypertrophy of the vital myocardium - the key elements of myocardium remodelling. A special highlight consists in appreciation of the mechanisms and tools of myocardial fibrosis and hypertrophy which happen during the period of post-infarction remodelling lasting 4-6 months. Likewise is emphasized the basis of pathological remodelling which is manifested by left ventricle dilation and worsening of heart failure. In regard to multi-marker panel applied in cardiovascular diagnosis and prognosis most significant predictors of post-infarction myocardium remodelling are unravelled.
В этой статье раскрываются важнейшие механизмы постинфарктного ремоделирования миокарда, а также присущие ему биомаркеры. Подчеркнуты патофизиологические события, произошедшие в острой фазе инфаркта, такие как воспалительная реакция и активация оксидативного стресса и металлопротеиназ внеклеточного ма-трикса, запускающих заживление некротической зоны и гипертрофию живова миокарда что являются основными элементами ремоделирования миокарда. Особое внимание уделяется оценке механизмов и инструментов фиброза и гипертрофии миокарда, которые происходят в период постинфарктного ремоделирования, продолжающегося 4-6 месяцев. Также подчеркивается основа патологического ремоделирования что проявляется расширением ле-вого желудочка и ухудшением сердечной недостаточности. В отношении мультимаркерной панели, применяемой в диагностике и прогнозе сердечно-сосудистой системы, наиболее значимые предикторы постинфарктного ремо-делирования миокарда не выявлены.
