Inflamația în patologia coronariană

Abstract

Inflamația, ca proces fiziologic, este cunoscută de mai mult timp, însă tangența cu patologia coronariană s-a făcut relativ recent. Deşi există mai multe teorii care ar explica legătura între ateroscleroză şi procesul inflamator, nu s-a stabilit un singur fir narativ care ar include toţi participanții cascadei inflamatorii şi ar aduce claritate. Astfel ne-am propus să studiem trialurile efectuate, materialele existente şi teoriile contestate, până în ziua de astăzi şi să conturăm şi să descriem un concept comun despre procesul inflamator în ateroscleroză, mai cu seamă în sindromul coronarian, explicând rolul fiecărui membru participant în acţiune.

Though many attempts to elucidate the correlation between atherosclerosis and the inflammatory process have been made, a single theory that would include all the participants of the inflammatory cascade has not yet been contoured. Despite the benefits of statin treatment as secondary prevention measures, many patients suffer from repeated coronary events, the greater part of them having elevated inflammatory markers. Most anti-inflammatory treatments have failed in clinical trials, rising the interest towards this subject and proving the necessity of further attempts. We tried to summaries the existing information and outline a unique concept about the inflammatory pro-cess in atherosclerosis, explaining the role of every participating member.

Несмотря на большое количество попыток установить точную связь между атеросклерозом и субклиническим воспалением, до сих пор отсутствует единая теория, которая смогла бы точно описать данную взаимосвязь и включить всех ее возможных участников в виде единого каскада. Невзирая на интенсивную терапию статинами с целью вторичной профилактики у пациентов с ишемической болезнью сердца, многие из них переживают повторные инфаркты или другие острые состояния и большинство пациентов, относящихся к этой наименее удачливой категории имеют повышенные маркеры воспаления. Большинство клинических исследований по тестированию противовоспалительных препаратов в дан-ной области не увенчались успехом. Мы попытались резюмировать известные данные о воспалении и атеросклерозе с целью создания более полной картины о взаимосвязи данных процессов и повышения интереса к этой очень важной проблеме.
