Abstract

Structura tezei: Textul tezei este expus pe 124 pagini de text de bază, procesate la calculator, fiind constituită din: lista abrevierilor, introducere, 4 capitole, concluzii generale, recomandări practice, bibliografia din 166 surse și 8 anexe. Materialul ilustrativ include 69 figuri, 1 tabel și 1 formulă. Rezultatele tezei au fost publicate în 13 lucrări științifice. Pe baza rezultatelor cercetărilor au fost obținute 1 brevet de invenție, 1 adeverință drept de autor și 2 acte de implementare în practică. Obiectivele studiului: Evaluarea particularităților manifestărilor clinice caracteristice gingivitei hipertrofice. Studierea particularităților morfologice în gingivita hipertrofică în dependență de factorul etiologic. Aprecierea efectului în timp a ozonului medical și utilizarea metodei funcționale de fluxmetrie cu laser Doppler în tratamentul gingivitei hipertrofice. Elaborarea unui algoritm optim în schema tratamentului complex al pacienților cu gingivită hipertrofică. Noutatea și originalitatea științifică: În premieră a fost utilizat ozonul medical în tratamentul complex al gingivitei hipertrofice, datorită proprietăților antibacteriene și antiinflamatorii cu diminuarea semnelor clinice de edem și hipertrofie gingivală, lichidarea pungilor gingivale și restabilirea țesutului gingival sănătos. În premieră a fost studiată minuțios și elucidată etiologia și patogeneza gingivitei hipertrofice cu evaluarea factorilor favorizanți și determinanți, particularitățile morfologice și funcționale și cele de tratament. În premieră s-a elaborat un algoritm optim în tratamentul complex al pacienților cu gingivită hipertrofică. În premieră pe un lot semnificativ de pacienți cu gingivită hipertrofică s-a realizat evaluarea la distanță a eficacității tratamentului complex cu ozon medical utilizând metoda funcțională de fluxmetrie cu laser Doppler la pacienți cu gingivită hipertrofică. Rezultatele obținute care contribuie la soluționarea problemei științifice importante: Administrarea locală a ozonului medical în tratamentul complex al gingivitei hipertrofice s-a soldat cu rezultate pozitive, eficiența fiind confirmată prin examenul clinic, paraclinic și funcțional. Astfel este soluționată problema apariției recidivelor în tratamentul gingivitei hipertrofice și prevenirea apariției complicațiilor. Semnificația teoretică și valoarea aplicativă: Datorită informației acumulate, proprietăților și efectelor descoperite pe care le exercită ozonul medical asupra țesuturilor, acesta s-a confirmat ca opțiune de succes în tratamentul gingivitei hipertrofice obținând rezultate pozitive în dinamică. Implementarea rezultatelor științifice: Кezultatele acestei lucrării științifice au fost implementate în cadrul Catedrei de odontologie, parodontologie și patologie orală „Sofia Sîrbu”, USMF „Nicolae Testemițanu” (act nr.2, din 22.01.2024; act nr.1, din 22.01.2024 ) și Clinica Stomatologică Universitară (act nr.1, din 22.01.2024).

Область изучения: 323.01 – стоматология Цель исследования: Изучение морфо-функциональных особенностей, диагностики и комплексного лечения гипертрофического гингивита с использованием медицинского озона. Задачи исследования: Оценка особенностей клинических проявлений, характерных для гипертрофического гингивита. Изучение морфологических особенностей при гипертрофическом гингивите в зависимости от этиологического фактора. Оценка влияния медицинского озона и использования функционального метода лазерного допплера при лечении гипертрофического гингивита. Разработка оптимального алгоритма в схему комплексного лечения пациентов с гипертрофическим гингивитом. Научная новизна и оригинальность: Впервые медицинский озон был использован в комплексном лечении гипертрофического гингивита благодаря антибактериальным и противовоспалительным свойствам с уменьшением клинических признаков отека и гипертрофии десен, ликвидацией десневых карманах и восстановлением здоровой ткани десны. Таким же образом были тщательно изучены и уточнены этиология и патогенез гипертрофического гингивита с оценкой благоприятствующих и определяющих факторов, морфофункциональных и лечебных особенностей. Таким образом, разработан оптимальный алгоритм комплексного лечения больных с гипертрофическим гингивитом. Впервые проведена дистанционная оценка эффективности комплексного лечения медицинским озоном с использованием функционального метода Лазерной допплерографии у больных с гипертрофическим гингивитом. Полученные результаты которые способствуют решению важной научной задачи: местное применение медицинского озона в комплексном лечении гипертрофического гингивита подтвердило положительные и эффективные результаты путем клинического, параклинического и функционального обследования. Таким образом, решается проблема рецидивов лечения гипертрофического гингивита и профилактики осложнений. Теоретическая значимость и прикладная ценность: благодаря накопленным сведениям о действии и свойствах медицинского озона на ткани это позволило его успешное использование в лечении гипертрофического гингивита с получением положительных результатов в динамике. Внедрение научных результатов: результаты научной работы внедрены на кафедре одонтологии, пародонтологии и патологии полости рта «София Сырбу», УМФ «Николае Тестемицану» (акт No2, от 22.01.2024; акт No1 от 22.01.2024) и Университетской стоматологической клиники (акт No1 от 22.01.2024).

Field of study: 323.01 – Dentistry The purpose of the research: Study of the morpho-functional features, diagnosis and complex treatment of hypertrophic gingivitis using medical ozone. Objectives of the study: Evaluation of the particularities of the clinical manifestations characteristic of hypertrophic gingivitis. Study of morphological features in hypertrophic gingivitis depending on the etiological factor. Appreciation of the effect of medical ozone and use of the functional method of laser doppler in the treatment of hypertrophic gingivitis. Elaboration of an optimal algorithm in the complex treatment of patients with hypertrophic gingivitis. Scientific novelty and originality: For the first time, medical ozone was used in the complex treatment of hypertrophic gingivitis, due to antibacterial and anti-inflammatory properties with the reduction of clinical signs of edema and gingival hypertrophy, the eradication of gingival pockets and the restoration of healthy gingival tissue. In the same way, the etiology and pathogenesis of hypertrophic gingivitis was thoroughly studied and elucidated with the evaluation of the favoring and determining factors, the morphological and functional features and those of treatment. Thus, an optimal algorithm in the complex treatment of patients with hypertophic gingivitis was elaborated. For the first time, the remote evaluation of the effectiveness of the complex treatment with medical ozone has been carried out, using the functional method Laser Doppler flow-metry in patients with hypertrophic gingivitis. The obtained results contributing to the solution of the important scientific problem: The local administration of medical ozone in the complex treatment of hypertrophic gingivitis led the positive results, and its efficiency was proved by the clinical, paraclinical and functional examinations. Thus, the problem of relapses in hypertrophic gingivitis treatment and prevention of complications has been solved. Theoretical significance and application value: due to the accumulated information, discovered features and effects of medical ozone on tissues, it has been proved as a successful option in treatment of hypertrophic gingivitis, achieving positive results in dynamics. Implementation of the scientific results: The results of this scientific work were implemented within the frame of Department of Odontology, Periodontology and Oral Pathology "Sofia Sîrbu", USMF "Nicolae Testemițanu" (act No2, dated 22.01.2024; act No1, dated 22.01.2024) and University Dental Clinic (act No1, dated 22.01.2024).
