Farmacistul clinician – scut în calea erorilor de medicație în farmacia de comunitate

Abstract

Introducere. Profesia de farmacist poate fi exercitată în multe sectoare de activitate, însă cel mai important rol al acestei profesii îl constituie furnizarea de servicii farmaceutice în folosul și spre binele pacientului (patient oriented). Încă de la apariția specializării de farmacie clinică s-a conturat influența majoră pe care o poate exercita farmacistul specialist asupra modului în care pacienții își urmează tratamentele prescrise de medic. Farmacistul clinician își folosește toate cunoștințele dobândite în învățământul universitar și postuniversitar pentru a stabili dacă planul terapeutic stabilit de medic este potrivit pacientului, în contextul numeroșilor factori care pot interveni în punerea în aplicare a schemei de tratament. De asemenea, farmacistul trebuie să reamintească periodic pacienților despre importanța instituirii, continuării și terminării tratamentului conform indicațiilor medicului prescriptor. Scopul lucrării. Ne-am propus să analizăm formularele de prescripții simple reținute în două farmacii de comunitate pe perioadă de două luni. Am urmărit identificarea erorilor apărute în scrierea acestor prescripții, analiza acestora, încadrarea lor în diverse categorii și modalitatea de gestionare de către farmacist. Material și metode. Am analizat un număr de 457 de prescripții simple, din două farmacii de comunitate din București. Am sortat prescripțiile după mai multe criterii și am cuantificat procentual tipurile de erori identificate. Rezultate. Din cele 457 de prescripții analizate, 380 au prezentat erori. Cele mai multe erori (85%) au fost cele tehnice, restul de 15% fiind erori științifice. Dintre erori putem menționa prescrierea medicamentelor în doză nepotrivită la copii sau prescrierea de medicamente care nu mai există în nomenclator. Farmaciștii clinicieni au avut intervenții prompte de remediere a erorilor în situațiile care au impus astfel de intervenții. Concluzii. Prezentul studiu reprezintă încă o dovadă privind rolul major al farmacistului clinician în farmacia de comunitate pentru depistarea, analizarea și remedierea erorilor de prescriere. Reiese clar necesitatea prezenței unui astfel de specialist în fiecare dintre farmaciile de comunitate.

Introduction. The profession of pharmacist may be practiced in many sectors of activity, but the most important role of this profession is the supply of pharmaceutical services to the benefit of the patient (patient-oriented). Since the emergence of clinical pharmacy specialization it has figured out the major influence that the specialist pharmacist can exert on the way patients undergo the treatments prescribed by the physician. The clinician pharmacist uses all the knowledge gained in university and postgraduate education to determine whether the physician’s plan is suitable for the patient, in the context of the many factors that may interfere with the implementation of the treatment regimen. The pharmacist should also periodically remind to the patients about the importance of setting up, continuing and stopping the treatment as prescribed by the physician. Purpose of the paper. The examination of simple prescription forms retained in two community pharmacies over a two-month period. Our purpose was to identify the errors that occurred in these prescriptions, to analyse and classify them, and to find out the way the pharmacist managed them. Material and methods. We analyzed 457 simple prescriptions from two community pharmacies in Bucharest. We sorted the prescriptions by several criteria and we quantified the types of identified errors. Results. Of the 457 analyzed prescriptions, 380 reported errors. Most errors (85%) were the technical ones, the remaining 15% being scientific errors. Among the mistakes we can mention there are the prescription of inappropriate doses in children or the prescription of drugs that no longer exist in the national formulary. Clinical pharmacists have had prompt interventions to correct the errors in situations that required such interventions. Conclusions. The present study is another evidence of the major role of the clinician pharmacist in the community pharmacy for detecting, analyzing and correcting prescribing errors. There is clearly a need for such a specialist in each of the community pharmacies.
