Managementul chirurgical al splenopatiei portale

Abstract

Rezumat Splenopatia portală (SP) reprezintă una dintre cele mai frecvente entităţi patologice în cadrul HTP prin ciroză hepatică, care pune nenumărate probleme legate de conduita terapeutică. Problema tratamentului pacienţilor cu SP a fost, și actualmente rămâne, una dintre cele mai importante în chirurgia HTP prin ciroză hepatică, în special la pacienţii cu hipersplenism sever. Concepţiile moderne asupra tratamentului chirurgical adresat SP urmăresc ameliorarea modificărilor fiziopatologiei HTP și sechelelor ei, supravieţuirea pacientului și ameliorarea calităţii vieţii. Summary Portal Splenopathy (PS) is one of the most common pathological entities in portal hypertension (PH) due to liver cirrhosis, which raises numerous problems related to therapeutic management. The problems of treatment of patients with PS has been and is currently one of the most important in surgery of PH by cirrhosis, especially in patients with severe hypersplenism. The modern concepts of surgical treatment addressed to the PS aim to improve the changes in the pathophysiology of PH and its sequelae, to overcome the patient and improve the quality of his life.
