ERORI DE MEDICAȚIE: TIPURI ȘI RECOMANDĂRI DE PREVENIRE

Abstract

Erorile de medicație reprezintă una din cauzele majore de mortalitate în lume. Conform datelor din literatură, majoritatea erorilor au apărut ca rezultat al complexității procesului terapiei medicamentoase, îndeosebi la etapa de prescriere. Farmaciștii comunică cu medicii prin intermediul rețetelor, iar analiza acestora permite de a depista erorile de prescripție. Studiul efectuat prezintă date despre incidența erorilor de prescripție în baza a 100 de rețete, ce au fost înregistrate în registrul rețetelor incorect prescrise, din 5 farmacii comunitare, timp de 16 săptămânii. Rețetele au fost analizate conform diverselor criterii. Conform analizei efectuate, medicii comit cel mai frecvent erori de prescriere legate de posologie, durata tratamentului și modul de administrare. În aceste cazuri, crește considerabil rolul farmacistului în evaluarea și monitorizarea medicației pacientului și necesitatea implementării recomandărilor de prevenire.

The medication errors are one of the major causes of mortality in the world. According to the literature, most of the errors occurred as a result of the complexity of medication process, especially at the prescribing stage. Pharmacists communicate with physicians through prescriptions, and their analysis allows to detect prescription errors. This study shows some data on the incidence of prescription errors based on 100 prescriptions, recorded in register of incorrectly prescribed prescriptions from 5 community pharmacies, during 16 weeks. Prescriptions have been analyzed according to various criteria. According to the analysis, physicians, most frequently, make errors related to the medicines dosage, the lasting of treatment and the way of administration. In these cases, the pharmacist’s role in assessing and monitoring patient medication and the need to implement prevention recommendations is considerably increased.
