Abstract: Modificările funcţiilor cardiovasculare ce apar în timpul anesteziei generale se datorează unor numeroşi factori, printre care efectele directe ale substanţelor anestezice. Au fost evaluate tendinţele moderne pentru asigurarea protecţiei intraanestezice a pacienţilor cardiaci gravi. Optimizarea balanţei cerere/ofertă de oxigen şi selectarea agenţilor anestezici, astfel încît oferta de oxigen să fie maximă, iar cererea de oxigen să fie cât mai mică – sunt cerinţe principale în conduita unui pacient cu cardiopatie ischemică. În stenoze valvulare este importantă menţinerea unor presiuni de umplere adecvată pentru a realiza un volum bătaie, debit cardiac corespunzător şi menţinerea ritmului sinuzal, se evită creşterea postsarcinii VD, tahicardia prin scurtarea diastolei având efecte hemodinamice negative. Pentru insuficienţe valvulare este importantă menţinerea presarcinii, frecvenţa cardiacă trebuie menţinută uşor crescută şi se evită creşterile RVS. La pacienţii hipertensivi este preferată anestezia cu anestezice volatile, doze mari de opioizi. La bolnavii cu tulburări de ritm şi conducere este de dorit ca aritmiile să fie minuţios controlate preoperator, la unele tipuri de blocuri cardiace este indicată implantarea unui pacemaker (cardiostimulator) temporar preoperator. Pacemakerele demand (cardiostimulatoare cu frecvenţă necontrolată – n.r.) sunt sensibile la câmpurile electromagnetice, ca cele produse de electrocauter care pot determina malfuncţie. În concluzie alegerea tipului de anestezie şi a substanţelor anestezic propriu-zise trebuie să fie făcută de un anestezist calificat, capacitatea de a monitoriza hemodinamica pacientului, de a reacţiona rapid, sunt mult mai importante decât substanţa anestezică propriu-zisă.
Changes in cardiovascular function that occur during the general anesthesia are due to many factors, including direct effects of anesthetic substances. There were assessed modern trends, to ensure intra-anesthetic protection to serious cardiac patients. Optimization of balance demand/supply of oxygen and anesthetic agents selected so that the supply of oxygen to be maximal and demand of oxygen to be as small as possible – are key requirements in the behavior of a patient with ischemic cardiopathy. In valvular stenosis it is important to maintain adequate filling pressures to achieve a volume of beat, appropriate cardiac flow and maintenance of the sinus rhythm, it is avoided the increase of the afterload in the right ventricle, tachycardia by diastole shortening having negative hemodynamic effects. For valvular deficiencies it is important the maintenance of preload, the cardiac frequency must be maintained slightly increased and it is avoided the RVS increase. To the hypertensive patients it is recommended the anesthesia with volatile anesthetic substances, high doses of opioids. To the patients with disorders of rhythm and control it is recommended that arrhythmias to be very well monitored presurgical, to some types of blocks it is recommended the implantation of a temporary presurgical peacemaker. Demand peacemakers are sensitive to electromagnetic fields, like those created by electrocautery which may determine the malfunction. In conclusion, the choice of the type of anesthesia and anesthetic substances has to be made by a qualified anesthetist, the ability to monitor the patient’s circulatory dynamics, to act quickly, are more important than the anesthetic substance itself.
