Abstract: Background. In the management of pregnant women with fractures of the musculoskeletal system, the mortality rate is increased, determining the timeliness and need to study trauma in pregnant women. Objective of the study. Optimizing the behavior of pregnant women with skeletal trauma. Material and Methods. Study of 24 pregnant women with trauma. The average age 25,2±1,9 years. Causes of trauma: road accidents 58,33%, trauma 16,66%, etc. - 25%. Found: multiple skeletal fractures with associated trauma (n=11); isolated trauma to the skeleton (n=13). Investigations: clinical examination; gynecological; USG; radiography, etc. The treatment performed was complex and individual. Results. Trauma structure: cranial trauma (n=5), thoracic trauma (n=8), abdominal trauma (n=3). Locomotor fractures (n=24): pelvic fractures (n=1), limbs (n=23). In the shock resuscitation department hospitalized 62,5% (n=15) pregnant women. Patients, according to the indications, underwent urgent, minimally traumatic surgeries: immobilization by plaster (n=8), installation of the device with external fixation (n=2); open repose, osteosynthesis of the thigh (n=2), lower limb bones (n=1); extrafocal osteosynthesis of the pelvic bones (n=1), etc. Conclusion. The particularities in the conduct of pregnant women with locomotor fractures consisted in rapid and accurate diagnosis of injuries with urgent surgery, minimally traumatic, which was stabilized the general condition of pregnant women and continuation of treatment. Introducere. În managementul gravidelor cu fracturi ale aparatului locomotor, rata mortalității este sporită, determinând actualitatea şi necesitatea studierii traumatismelor la gravide. Scopul lucrării. Optimizarea conduitei gravidelor cu trauma scheletului. Material și Metode. În studiu sunt incluse 24 de gravide cu traumatism. Vârsta medie –25,2±1,9 ani. Cauzele traumatismului: accidente rutiere 58,33%, сatatraumatism 16,66%, ş.a 25%. Constatate: fracturi multiple ale scheletului cu traumatism asociat (n=11); trauma izolata a scheletului (n=13). Investigaţiile: examenul clinic; ginecologic; USG; radiografia, etc. Tratamentul efectuat a fost complex şi individual. Rezultate. Structura traumatismului: traumatism cranian (n=5), al сutiei toracice (n=8), trauma abdomenului (n=3). Fracturi ale aparatului locomotor (n=24): fracturi pelviene (n=1), membrelor (n=23), etc. În secţia de reanimare în stare de şoc internate 62,5% (n=15) gravide. Pacientelor, conform indicaţiilor, le s-au efectuat intervenţiile chirurgicale urgente, minim traumatice: imobilizarea prin gips (n=8), instalarea dispozitivului cu fixare externă(n=2); repoziție deschisă, osteosinteza coapsei(n=2), oaselor membrelor inferioare (n=1); osteosinteza extrafocală a oaselor pelvine (n=1), etc. Concluzii. Conduita gravidelor cu fracturi ale scheletului a constat în aprecierea corectă a stării pacientei, diagnosticul rapid, precis al leziunilor cu efectuarea intervenţiilor chirurgicale urgente, minim traumatice, care a permis stabilizarea stării generale a gravidelor şi continuarea tratamentului.
