Abstract: | Summary:
During 1995-2012 period 83 patients with CD were treated.
The localization of inflamation was: terminal ileitis – 24
(29.1%), colon – 9 (10.8%), ileum and colon – 16 (19.3%),
perianal region 34 (40.8%) cases. The distribution, according to CDAI Best was: easy =150-300/ 28 (33.7%),
moderate =301-450/ 35 (42.2%), severe > 450 points/ 20
(24.1%) cases. 38 (45.7%) from 83 patients underwent surgical treatment. Indications for surgery: ileocolic CD with
vital complications – 8 (21.1%) obstructive strictures – 5
(13.2%), internal and external fi stulas – 4 (10.5%), severe
steroid-linked effects – 3 (7.9%) cases; Perianal CD with
abcesses, fi stulas, strictures – 18 (47.2%) patients. Types
of surgical interventions: Ileocolic CD with segmentar
resection+anasthomosis – 10 (26.3%), plasty of stricture – 3
(7.9%), colectomia – 7 (18.4%); perianal CD with – rectal
resection+anasthomosis – 3 (7.9%), draining of the abscess
– 3 (7.9%), draining of fi stula by Seton – 5 (13.2%), incision
and plasty of stricture – 3 (7.9%), fi stuloplasty with vaginal
flap – 2 (5.3%), ileostomy – 2 (5.3%) cases.
Ileocolic CD: recurrence of illness – 3 (15%), repeated
intervention – 2 (10%), postoperative complications – 4
(20%), postoperative lethality – 1 (5%) cases. Perianal
CD: recurrence of illness – 6 (33.3%), repeated intervention – 4 (22.2%), postoperative complications – 5 (27.7%),
postoperative lethality – 1 (5.6%) cases. Резюме:
На протяжении периода исследования (2000-2012) было
госпитализировано 83 больных с болезнью Крона (БК).
Локализация болезни была следующей: терминальный
отдел подвздошной кишки – 24 (29.1%), толстая
кишка – 9 (10.8%), тонкая или толстая кишка – 16
(19.3%), перианальная область и промежность – 34
(40.8%). По индексу Беста активности заболевания
(CDAI) выявили: легкую форму = 150-300 у 28 (33.7%),
среднетяжелую форму = 301-450 у 35 (42.2%) и тяжелую форму > 450 у 20 (24.1%). Показаниями для оперативного вмешательства при кишечной локализации
болезни Крона послужили: жизнеопасные осложнения
– 8 (21.1%), обструктивные стриктуры – 5 (13,2%),
внутренние и наружные свищи – 4 (10.5%), стероидная
резистентность и зависимость – 3 (7.9%); при перианальной локализации: абсцессы, свищи, стриктуры – 18
(47.2%). Было произведено 10 (26,3%) сегментарных
резекций с анастомозом, 3 (7,9%) cтриктуропластик,
7 (18,4%) колэктомий, при перианальной БК – резекция
прямой кишки с анастомозом – 3 (7,9%), дренирование
абсцесса – 3 (7,9%), дренирование свища по Сетону – 5
(13,2%), стриктуротомия со стриктуропластикой – 3
(7,9%), пластика свища влагалищным лоскутом – 2
(5,3%), илеостомия – 2 (5,3%) случаев. |
