Abstract: Thyroid hormones have relevant effects on the cardiovascular system. Thyroid hormone-induced changes in cardiac function in patients with hypo- and hyperthyroidism can result from direct or indirect effects. Direct effects result from triiodothyronine (T3) action in the heart itself and are mediated by nuclear or extranuclear mechanisms. Nuclear T3 effects are mediated by the binding of T3 to specific nuclear receptor proteins, which results in increased transcription of T3-responsive cardiac genes that encode both structural and functional proteins. Extranuclear effects influence primarily the transport of calcium, amino acids and sugars across the cell membranes. Also T3 influences the sensitivity of the sympathetic system and induces hemodynamic alterations in the periphery that results in increased cardiac filling and modification of cardiac contraction. Hormonii tiroidieni posedă efecte relevante asupra sistemului cardiovascular. Modificările funcţionale ale miocardului prezente la pacienţii cu hipo- şi hipertiroidie apar graţie efectelor directe sau indirecte ale hormonilor tiroidieni. Efectele directe sunt consecinţa acţiunii nemijlocite a triiodtironinei (T3) asupra cordului şi sunt mediate de mecanisme nucleare şi extranucleare. Efectele nucleare sunt realizate prin fixarea T3 la receptori nucleari specifici ce conduce la sporirea transcrierii genelor cardiace T3-dependente codificatoare de proteine structurale şi funcţionale. Efectele extranucleare influenţează prioritar transportul calciului, aminoacizilor şi al glucozei prin membranele celulare. De asemenea, T3 influenţează sensibilitatea sistemului nervos simpatic şi induce alterări în hemodinamica periferică, urmate de creşterea sensibilităţii cardiace şi modificarea contractilităţii miocardului.
